Accident: 784728 - Killed When Crushed In Transfer Conveyor
At approximately 9:45 a.m. on July 21, 2000, Employee #1 was operating a transfer conveyor that jogged up and down to move lumber. A scrap of wood became jammed in the machine and he reached through the framework of the conveyor to clear it and to activate the machine's switch. The conveyor raised up and caught the midsection of Employee #1's body. He sustained crushing injuries and died the following day during surgery.

Accident: 170250005 - Killed When Truck Rolls Down Embankment
Employee #1, age 20, was driving up a hill when he lost control of his truck. He went over an embankment and the truck rolled over several times. Employee #1 was treated by EMS and underwent emergency surgery at Okanogan/Douglas Hospital. Once he was stabilized, he was airlifted to Deaconess Hospital in Spokane, WA, for further treatment. Employee #1 died later that night, having suffered massive blood loss and severe abdominal injuries.

At approximately 12:08 p.m. on April 4, 2000, Employee #1 was moving an I-beam when it was struck on the opposite end by another I-beam as it exited the automatic saw. Employee #1's beam went into his abdomen, and it lifted and pushed him into a shelf rack and up against stacked hangers. Employee #1 became caught between the rack and the beam; he sustained injuries that required hospitalization.

At approximately 12:00 noon on February 22, 2000, Employee #1 was flagging eastbound traffic at milepost 84 on State Highway 12. He was struck by a 1996 Chevrolet Cavalier driven by a 73-year-old woman. Employee #1 suffered trauma to the head, neck, abdomen, and chest, and was killed.

Employees #1 and #2 both started work at approximately 8:00 a.m. At 10:20 a.m., both were shot by an unknown gunman. Employee #1 was shot in the chest and abdomen; he was transported to Harborview Trauma Center, where he died at 12:50 p.m. Employee #2 was shot several times and died at the scene at 10:30 a.m. These were Medical Examiner's cases #99-1246 and #99-1247, and police case #99-485794.

On July 29, 1999, Employee #1, a sawyer for a manufacturer of wood trusses, was using a short 2-by-4 to stop a web saw blade that was winding down. The board caught a bolt head on the blade and was kicked back. It struck him in the abdomen and knocked him down. Employee #1 sustained internal bleeding, for which he was hospitalized.

At approximately 4:20 p.m. on July 3, 1999, Employee #1 was working as a laborer for Tom's Welding & Fabrication in San Francisco. He and his coworkers were carrying a 6 ft C-channel to the second floor of a school, with Employee #1 at the lowered end. As the channel was being brought up the stairway, he slipped and fell. The workers dropped the channel and it slid downward, landing on top of him. Employee #1 suffered internal abdominal injuries and was transported to San Francisco General Hospital.

At approximately 3:10 p.m., Employee #1, a Metro transit bus driver, was headed south on the second trip of his normal route. He was crossing a bridge when he was shot in the abdomen by a passenger. He lost control of the bus, which broke through the bridge barrier, hit the roof of an apartment, and came to rest approximately 20 to 30 feet below the bridge deck. Employee #1 was ejected from the bus and landed on the apartment roof. He was pronounced dead at 3:30 p.m.

Employee #1 and five coworkers were tunneling in an approximately 22 to 24 ft deep, 8 to 10 ft wide trench. The day before, a mechanic from the company had replaced a broken 3/4 in. male nipple along with a 3/4 in. male/female quick connect coupling, part #S10S-63500. The parts were installed on the hydraulic jack of a pneumatic clay spade, and the equipment was functioning properly after installation. While in operation the next day, the hydraulic line split laterally from the nipples and broke, striking Employee #1 in the chest and abdomen. He was killed. Trench boxes were being used.

At approximately 5:15 p.m. on May 29, 1998, Employee #1 was operating a Sidney lathe when the safety block and driver system flew off the face plate. The driver struck him in the chest and abdomen, while the safety block was thrown approximately 141 ft and hit a concrete block wall. Employee #1 was killed. The employer designed, manufactured, and used a non-industry standard lathe dog drive and safety block system, which allowed the device to fly apart under either normal or emergency conditions.

Accident: 948786 - Killed When Stuck By Falling Beam
At approximately 9:00 a.m. on April 29, 1998, Employee #1 was helping the foreman rig a 32 ft 6 in. long I-beam with a 5 ft 7 in. long nylon sling. Before they erected the beam it had to be turned, so Employee #1 tied a tag line on the beam to control its movement. When he completed turning the beam, he climbed onto the east side of the crane operator's platform. Employee #1 lost his balance and pulled down the tag line, causing the beam to dip and slip out of the nylon sling. The beam slid down to the operator's station, striking Employee #1 in the abdomen and knocking him from the operator's station. Employee #1 was transported to the hospital, where he was pronounced dead on arrival.

At approximately 6:15 p.m. on December 19, 1997, Employee #1was walking under the elevated bucket and boom of a Thomas front end loader, model T233HD, serial #LH 000507. He was facing the loader, with his torso near the controls and his feet on the ground, when the bucket and boom suddenly came down, pinning him against the frame. He died of abdominal and neck injuries due to blunt force trauma and compression. Employee #1 had greased the front end loader just before the accident.

Employee #1 was rushed to the hospital, but died in surgery from crushing injuries to his chest and abdomen.

He was trying to manually remove a cap when an explosion occurred. Employee #1 was splashed by the residual caustic soda solution (50 percent by weight), and suffered chemical burns to his abdomen, face, genitals, and eyes.
