Accident: 119958858 - Two Employees Injured In Fall When Crane Boom Fails
At approximately 7:24 a.m. on August 12, 1997, Employees #1 and #2 and two coworkers were engaged in a large commercial sign replacement project. They were using a Baldwin-Ward truck-mounted crane to access the sign structure, and were at a height of approximately 70 ft when a segment of the telescoping boom failed. Employees #1 and #2 were riding with the sign and sustained injuries when they fell. Employee #1 was transported to Bullhead Community Hospital, where he was treated and released. Employee #2 was transported by air ambulance to the University of Las Vegas Medical Center, where he was admitted for treatment.

Accident: 170082580 - Electric Shock - Direct Contact With Overhead Line
A maintenance worker for a sign manufacturer was operating an aerial ladder mounted on the bed of a truck. While he was working, he contacted an overhead power line. He received an electric shock, which caused him to fall to the ground. The employee was taken to a hospital, where he died of electrocution 9 hours after the accident.

An employee was working on a billboard. He contacted a 12-kilovolt overhead power line that was about 2.1 meters away from the structure. He was electrocuted. He also fell about 6.1 meters to the ground.

Accident: 201040144 - Killed In Fall Through Decking Inside Marque
Employee #1 was replacing the marquee for upcoming attractions at a Hotel and Casino. He was tightening the remaining loose straps inside the bottom of the cube in the enclosed area when he apparently lost his footing on the steel braces and landed on the corrugated metal decking. The decking came apart and Employee #1 fell approximately 140 feet onto concrete. He was killed.

Accident: 201770088 - Electric Shock - Ground Fault In Electric Sign
An employee was reassembling panels on a McDonald's sign that had just been refurbished. A second employee, the supervisor, plugged the sign in to check the proper operation of the lights. The first employee was electrocuted by an apparent ground fault in the sign. He had one hand on one of the metal arches when the supervisor energized the sign.

Accident: 201030186 - Electric Shock - Cause Unknown
Two service technicians and their service manager arrived at a jobsite with a Skyhoist remote control aerial lift to repair a sign. The 3-meter-by-3-meter-by-3-meter cubic sign was located 7.6 meters above the ground. It had been raining, and water had accumulated inside the sign. One of the service technicians went up in the aerial lift at the east end of the sign and exited onto the sign. He walked over to the 457-millimeter-by-457-millimeter hatch on the opposite side of the sign, which was about 3.4 meters from an overhead power line. He was using the vehicle-mounted generator on the aerial lift to provide temporary electric power. The service technician went inside and drove away some pigeons. Afterwards, his coworkers observed flashes inside the sign. The coworkers shut off the generator and called the injured employee's name. When they got no answer, the remaining service technician went up the ladder on the aerial lift and discovered the injured employee's body on the roof of the sign. He had been electrocuted.

Accident: 200850121 - Killed In Fall From Billboard Platform
At 1:30 p.m. on July 1, 1996, Employee #1, a billboard sign and poster painter, and a coworker were working on a billboard. Employee #1 had just finished pasting a portion of the sign and was on the main platform of the structure when he fell 26 ft onto the flat roof of a building. He was killed. The main platform of the billboard did not have railings. Employee #1 had on a safety harness and there was a horizontal safety wire strung along the main platform but he was not tied off to it. The employer was cited for failure to provide railings and failure to ensure workers used personal fall protection equipment.

Accident: 170606917 - Injured In Fall From Billboard
At approximately 10:00 a.m. on May 2, 1996, Employee #1 was on the work platform of an advertising billboard. He was using a safety harness with a lanyard for fall protection, and the work platform was equipped with a single guardrail. Employee #1 unfastened his safety lanyard and was moving about the work platform when he fell approximately 18 feet onto a rooftop. He sustained compound fractures of his right arm and wrist, and severed several tendons, for which he later underwent surgery. The employer's records showed that Employee #1 had received training in the required use of a safety harness and lanyard.
