  • Accident: 170041206 - Dies After Suffering A Heart Attack

    At approximately 1:40 a.m. on June 9, 2000, Employee #1 was working on the weigh-and-price line at the Foster Farms poultry processing plant in Kelso, WA. She was responsible for transferring packages of wrapped poultry products from storage trays to a conveyor line for final packaging. Employee #1 began suffering severe chest pains and slumped to the floor. Nearby coworkers immediately called for medical attention. An onsite EMT responded and began first-aid and CPR until Emergency Services arrived and took over. Employee #1 was transported by ambulance to St. John's Hospital in Longview, WA, where she was pronounced dead on arrival due to an apparent heart attack. Employee #1 had a prior history of heart disease and high blood pressure. The autopsy report indicated that Employee #1 died of a dissecting aneurysm of the ascending aorta. There were no work-related factors contributing to her death.

    Accident: 170385595 - Dies Of Aneurism After Fall From Roof

    Employee #1, a steel worker, and two coworkers were on a 22 ft high building, installing insulation and steel roof deck sheets. They unrolled insulation for a row of sheeting. The wind was blowing the insulation, so Employee #1 removed a sheet of roof deck from a stack of two sheets that was being used as a leading edge walkway and put it on the insulation to hold it in place. He then stepped back onto the walkway, which was now only one sheet thick. The roof decking buckled under his weight, and he fell 22 ft to the concrete floor. Employee #1 was hospitalized for six days and his condition was improving, when he died of a massive aneurism deep within his brain.

    Accident: 170008221 - Dies Of Aneurysm

    Employee #1, age 47, was assembling a transformer when he complained that he was not feeling well. He was followed by the company owners as he went to the lunch room to rest at the end of the his shift. Employee #1 collapsed and emergency services was called. Employee #1 died of a ruptured aorta, or aneurysm; his death was attributed to natural causes.

    Accident: 170238380 - Dies Of Brain Aneurysm

    Employee #1 had been performing his normal duties when he was found, unresponsive, lying on some boxes. He apparently died of a ruptured neural aneurysm.

    Accident: 201380078 - Aneurysm

    A 22-year-old telephone technician was installing an extension for an additional telephone in the living room of a residence. He was found on the floor of the basement by the occupant of the home. The occupant called emergency services, and the police and an emergency medical unit responded. The emergency medical technicians tried to revive the employee, but their efforts were unsuccessful. The employee was declared dead on arrival at the hospital. The autopsy report indicated that the cause of death was cardiac arrest caused by an ulcerated plaque (aneurysm).

    Accident: 200690048 - Dies Of Aneurysm That Led To Heart Failure

    Employee #1 complained about a "burning chest," while he was at work. He was driven home by a coworker and then taken to the hospital by his parents. A physician there called Employee #1's workplace and asked for the Material Safety Data Sheets (MSDS) for any chemicals to which the worker might have been exposed. The doctor assessed this information and felt that Employee #1's condition was not related to chemicals. He was released from the hospital and driven home by his parents. Shortly after Employee #1 arrived home, he passed out. An ambulance was called and Employee #1 was transported to the hospital, where he was pronounced dead. The cause of death was an aneurysm at the beginning of the aorta, which caused heart and circulatory system failure.

    Employee #1 was killed by a ruptured aneurism in the brain.

    Accident: 170040950 - Dies Of Ruptured Aneurism

    At approximately 11:00 p.m. on March 18, 1995, Employee #1 became ill during his second shift. He reported to the medical center on site and was transported to the emergency room at the local hospital, where he was observed and sent home. He died at home five days later. He was treated for a viral illness, but the cause of death was a leaking aneurism. Employee #1 had been on blood pressure medication until February, at which time he monitored his blood pressure at work and stopped medication. He had had some exposure to low frequency noise and chemical slimicide at the wet lap machine, but it was determined that no safety standards were violated.
