  • Accident: 170080337 - Injured When Struck By Pipe, Dies In Surgery

    Employee #1, a longshoreman, was in the hold of a barge as bundles of pipe were being off loaded. A nylon sling broke and the load of pipes swung over, striking Employee #1 on the legs. He was transported to the hospital, where he underwent surgery. During the operation he suffered an apparent blood clot or heart attack, and died.

    Accident: 170082366 - Drowns After Falling Off Barge

    Employee #1, a utility worker at a bulk loading marine terminal, was attempting to open a cover on a hopper barge when he apparently slipped off the hopper cover or the deck of the barge. He fell into the water and drowned. There were no witnesses to the accident. Autopsy reports showed he struck his head before falling into the water. He was wearing a Coast Guard approved work vest; however, he had become caught underwater by a wire rope cable beneath the dock.

    Accident: 301309589 - Killed When Crushed Between Cargo Containers

    Employee #1 was loading cargo containers onto a barge when he became caught and was crushed between two containers. He then fell to the floor of the barge. Employee #1 was killed.

    Accident: 14509301 - Drowns After Falling From Barge

    Employee #1 apparently fell off a flatbed barge into the water and drowned. There were no witnesses to the accident. His body was found in the area used to access the barge.

    Accident: 170710040 - Killed After Falling Off Barge During Storm

    During the course of their work, a crew had placed a barge on the south side of an existing dock. At high tide at approximately 2:30 p.m., the weather began to deteriorate and the crew decided to move the barge into deeper water on the north side of the dock. Employee #1, who had been working on shore, jumped onto the barge for a ride. While the barge was in motion, a bolt of lighting struck close by. The crew looked up at the sky and when they looked back, they saw Employee #1 falling into the water. He was killed. Employee #1 was not wearing a personal flotation device.
