Employee #1 was applying a roof sealer on a flat roof. He was walking backward when he went through a skylight and fell 32 ft to the ground. Employee #1 was killed.

Accident: 898817 - Killed In Fall Through Stair Opening
At approximately 10:30 a.m. on December 27, 1999, Employee was working on the second floor of a house under construction. He was unrolling Tyvex plastic sheeting on a 9 ft high by 40 ft long wall that had been built on the second-floor deck. The wall was still laying on the deck as Employee #1 walked backward, leaning over to unroll the sheeting. He stepped through a window opening in the wall that was directly over an uncovered stairway opening in the deck. Employee #1 fell through the second-floor stair opening to the concrete basement floor. He was killed.

Accident: 170243745 - Killed In Fall Through Cut-Out Roof Section
At 10:50 a.m. on December 22, 1999, Employee #1 and five to seven coworkers were on a low-pitched roof. They were off loading some materials onto the roof and Employee #1 was asked to retrieve a scrap of wood to chock a roll of material. He walked quickly to get the chock and was headed directly for a 6 ft by 8 ft section of roof that had just been scored for an elevator shaft. When he was about 6 ft away from the cut area, the roof safety monitor called for him to stop, but a loud generator next to the cut-out section may have drowned out the warning. Employee #1 stepped onto the cut section, which buckled, and he fell 30 ft to the bottom of the shaft. He suffered massive head trauma and neck injuries. A doctor at the site administered first aid and Employee #1 was rushed by emergency personnel to the hospital, where he died.

Accident: 897652 - Killed In Fall Through Skylight Opening
Employee #1, age 20, and coworkers were dragging sheets of metal roof decking to the point of installation. To try to complete a section before quitting time, the majority of workers were instructed to move the decking to the proper location. Employee #1 set down a sheet of decking and stepped backward through an opening where a skylight would later be installed. He fell 21 ft 8 in. onto a concrete floor. Employee #1 was killed.
