  • Accident: 101571446 - Killed When Crushed By Falling Tanker Trailer

    At approximately 11:30 a.m. on September 7, 2000, Employee #1 and the company co-owner were called to a scene where one of their truck drivers was delivering hot emulsion material to a construction site. The tanker became detached from the tractor, and he called the company office to report this incident because the tanker was blocking an active street. A tow service was called to help lift the front of the tanker so that it could be place back on the tractor. It was decided that the tanker was to heavy to lift and that some of the emulsion would need to be removed. A coworker and the co-owner drove to the site with a second tanker, which was going to be used to off-load the damaged tanker. After about half of the contents of the damaged tanker had been emptied, a tow truck attempted to lift the front end so the hot emulsion would flow out the rear outlet. For this process, 4-by-4 wood blocks were placed between the pavement and the tanker to "crib" it. Once the blocks were in place, Employee #1 decided to crawl on his stomach with a chain to try and secure the crane to the fifth wheel pin. As he was crawling under the tanker, his coworkers called, "Don't go under there." The tanker collapsed onto Employee #1 and he sustained multiple injuries, from which he died a short time later.

    Accident: 801175 - Killed When Pinned Under Bus

    Employee #1, a wrecker operator, had come from Missouri to tow back a bus. He used his toe truck to raise the bus and placed two 4-by-4 in. boards under each front tire to allow him to connect his tow truck properly to the bus. Employee #1 then went under the bus to hook up his wrecker to the front axle. The location of the bus had received a lot of rain in the past week and the 4-by-4 blocks began to sink and turn sideways. This caused the bus to slip off the blocks, pinning Employee #1. He died of suffocation.

    Accident: 170564926 - Crushed When Dump Truck Shifts Forward

    Employee #1 was placing a dump truck on wooden blocks so that he could reposition the tow truck hoist. The dump truck suddenly shifted forward, crushing him between the two vehicles. Employee #1 was killed. One or both of the blocks may not have been centered under the tires, which may have caused the truck to shift.

    Accident: 170150726 - Hand Crushed By Falling Flatbed Rail Car

    Employee #1 was working on an axle of a flatbed rail car, which had been raised using a hydraulic jack. It was also chocked on opposite sides by multiple blocks of wood, which did not effectively block or support the load. A crane lowered another flatcar onto the track adjacent to the car on which Employee #1 was working. When the flatcar was set down, it rolled and struck a remixer car, which then bumped the flatbed car and knocked it off the jack. Employee #1 sustained a laceration when the derailed car came down on his hand.

    Accident: 170609002 - Leg Crushed By Falling Equipment

    On January 3, 1998, Employee #1, the operator of a lifting conveyor at a corrugated container manufacturing facility, was standing nearby while it was repaired. Maintenance personnel were fixing the lifting chin when the unblocked feeder table fell, crushing his right leg and foot.

    Accident: 170238455 - Killed When Crushed By Falling Compressor

    Employee #1 was repairing the bearings on a mobile air compressor. He had jacked up the machine and gone under it to knock off the bearings when the compressor fell off the jack, landing on his head and neck. Employee #1 was killed. No cribbing or blocks were used to ensure the compressor did not fall.

    Accident: 170888093 - Injured In Fall When Extension Ladder Slips Out

    Employee #1, of Construction Sheetrock Work, was repairing holes in an interior sheetrock wall and preparing to install new sheetrock at a small industrial complex. He climbed an approximately 14 1/2 ft tall aluminum extension ladder to trim ragged hole edges in the sheetrock. The ladder slipped and Employee #1 fell onto the concrete floor, fracturing his left ankle and right heel. He was using an unknown brand of extension ladder that he had taken from a "pile of ladders" lying next to the work area. It was damaged and had no safety feet, nor was it blocked or tied off to prevent displacement. The employer had provided an extension ladder in good condition, which Employee #1 had on his truck at the time of the accident.
