  • Accident: 200640498 - One Killed, One Injured In Tire Explosion

    At approximately 1:30 p.m. on February 28, 1997, Employees #1 and #2, both tire repair contractors in a plant's fleet maintenance shop, had just completed repairs to the right front tire of a Caterpillar 910 front end loader, serial #40Y3083. Employee #1 had inflated the tire to approximately 40 psi. He was picking up his tools when Employee #2 added more air to the tire, resulting in an almost immediate explosive separation of the multi-piece wheel. The flange ring flew off and struck #2, killing him. The tire struck #1, who sustained minor injuries.

    Accident: 200600021 - Killed When Struck By Flying Tire Rim

    #1 was mounting a tire on a truck when the tire blew out. He was struck by the rim and died on February 11, 1997.

    Accident: 171017197 - Injured When Tire Explodes During Inflation

    #1 went to a construction site to repair a flat tire on a rough terrain forklift. He removed the tire from the machine, installed a new tube, and then propped the tire up against the forklift. When he inflated the tire slightly he noticed that the lock rings, which were located on its back side, did not seem to be in the correct position. #1 let the air out of the tire and pounded the lock rings into what he thought was the proper configuration. He then re-inflated the tire, this time to between 34 and 40 lb, at which point the tire blew apart and struck him on the legs. #1 was transported to Albany General Hospital, where he was treated for a fractured pelvis, compound fractures to both legs, and a concussion.

    Accident: 200230126 - Killed When Loader Overturns

    #1 was using a 1968 Caterpillar 966C wheeled loader to move a pile of brush out of a logging road under construction. He was driving up a slight incline when the left front tire blew out and the loader overturned, crushing the cab. #1 suffered a broken neck and was killed. The cab lacked roll-over protection, which was not required on vehicles manufactured before July 1969.

    Accident: 170100549 - Dies When Truck Overturns After Tire Blowout

    At 8:14 a.m. on September 20, 1996, #1 was driving a 1989 Peterbilt truck on his normal route, picking up products at assigned businesses. #1 was southbound in the left lane of Highway 167 near Auburn, WA, when his left front tire blew. #1 was unable to maintain control and the truck overturned on the driver's side into the median, pinning him underneath it. #1 was hospitalized and died on September 23, 1996.

    Accident: 170833594 - Two Injured When Struck By Tire Sidewall

    Employees #1 and #2 were inflating a tire (not marked Lt) when the side wall of the tire blew out. The exploding tire bruised #1's chest and bruised Employee #2's groin. Both were taken to the hospital and released that day. The tire, with its single piece rim, had not been placed in the available tire cage. There were no work rules regarding single piece rim wheels for dump trucks. No training had been provided to the two employees.

    Accident: 170812051 - Person Injured In Tire Explosion

    On November 17, 1994, a person was working on a tire and wheel at Sal Colledo Garage in Roseville, CA. He was not an employee, but an acquaintance who had been allowed to use the equipment in the shop to change tires. He had mounted four tires on wheels and was in the process of filling one of the tires with air as it lay on the concrete floor. The tire and wheel were not restrained by a tire machine. While it was filling, the tire blew out, sending the tire and wheel flying up into the face and arm of the person. He received multiple breaks of the right arm and severe facial lacerations. He was given first aid by the Roseville Fire Department paramedics and was transported to Roseville Community Hospital.
