  • Accident: 112540901 - Drowns In Lake

    At approximately 3:30 p.m. on November 16, 2000, Employee #1 was attempting to remove a 40 ft pontoon boat from a lake for winter storage. He had backed a truck and boat trailer down the boat ramp into the water, and they rolled out approximately 40 ft into the lake. Employee #1 drowned while trying to swim to shore. His body was found in 12 ft of water, and the truck and trailer ended up in 15 ft of water. No personal flotation devices were available on either the truck or boat.

    Accident: 170574305 - Killed When Boat Overturns

    Employee #1 was test driving a customer's boat when it flipped over. He suffered multiple injuries to his neck and head, and was killed.

    Accident: 100450766 - Killed In Fall Into New Fuel Tank

    Employee #1, a welder, was found lying on his left side at the bottom of a fuel tank in a newly constructed crew boat. Apparently he slipped while descending and fell against some piping. Employee #1 was killed.

    Accident: 170040356 - Drowns When Surf Rescue Boat Capsizes

    Employee #1 was the team leader of a police surf rescue team. The police received a 911 call regarding a surfer in distress and dispatched Employee #1's team to the scene. Two coworkers launched a jet ski to assist the surfer, but a large wave capsized it and threw the coworkers into the water. Employee #1 and a third coworker then launched a 13 ft Achilles surf rescue boat to assist the two coworkers and the surfer in distress. Another large wave capsized that boat, throwing Employee #1 and the third coworker into the water. Employee #1 drowned. He was not wearing the proper personal protective gear. In addition, the motor on the boat had not been properly maintained; it had water in the gas tank and the spark plugs were worn out.

    Accident: 200150100 - Drowns After Falling From Lobster Boat

    On August 1, 1997, Employee #1, the deckhand on a lobster boat, was re-setting 20 traps stored on the transom of a boat. There was 50 to 75 ft of polyethylene line between each of the traps. Employee #1's foot became entangled in the line on deck and, as the last of the traps went in the water, he was pulled overboard. Employee #1 drowned.

    Accident: 170993067 - Drowns In Fall From Tugboat

    At approximately 11:21 a.m. on December 11, 1996, Employee #1 was walking on a tugboat that was pushing two empty barges in San Pablo Bay from Tiburon to Glen Cove, CA. He fell off either the tugboat or one of the barges near Davis Point, CA. The captain of the tugboat heard Employee #1 yelling that he could not swim and threw him two life rings, but Employee #1 did not grab them. The captain stopped the tugboat and dove into the bay, but Employee #1 went under before the captain could reach him. Although his body was not recovered, he was presumed to have drowned. Employee #1 was not wearing a buoyant vest at the time of the accident.

    Accident: 201770120 - Dies In Fall From Moving Vessel.

    Employee #1 was servicing the navigation and running lights on a boat when he fell from the moving vessel and/or dredge piping that was being pulled. He died, but the cause of death was not determined.

    Accident: 569558 - Drowns In Attempt To Swim From Overturned Boat

    Employee #1 and five coworkers were crossing a stream when the waves from a passing barge swamped their small aluminum boat. All employees stayed with the overturned boat, except Employee #1, who attempted to swim to the shore. Before he reached shore, the swift undercurrent of the stream pulled him under and he drowned. There were no life preservers in the boat; all life preservers had been left behind in another boat.

    Accident: 170993141 - Killed After Falling From Boat

    At approximately 7:15 p.m. on September 17, 1996, Employee #1 was performing her duties as morale, welfare, and recreation marina employee. She took a 17 ft Boston Whaler V-hull powerboat to check on two divers that had failed to return to the marina or respond to several radio calls. She went to the north island dock, made contact with both divers, and then left to return to the marina. Shortly thereafter, the divers left the dock for the marina and noticed Employee #1's boat, unattended, circling at full speed. A search for the body was conducted and discontinued on September 19, 1996. The Air Force Office of Jurisdictional Service, Hickam AFB, concluded that there was no foul play.

    Accident: 14532758 - Drowns Trying To Disentangle Fishing Net

    At approximately 10:30 p.m. on July 4, 1996, Employee #1 was working aboard the fishing vessel Tiffany Lee, which was participating in a salmon opening (purse seiner) at Raspberry Island, Kodiak, Alaska. The fishing net became caught on the ship's propeller. Employee #1 dove into the water to free it, but became entangled in the net. A distress call was broadcast, and vessels in the area responded with divers and rescue skiffs. Employee #1's body was recovered around 2:00 a.m. on July 5, 1996. He drowned.
