Accident: 700682 - Killed In Fall From Microwave Tower
Employee #1 was climbing down an 80 ft high microwave tower under construction to break for lunch. He was at a height of approximately 60 ft when a crane operator noticed that a diagonal piece of the cross-arm swing out and strike him. Employee #1 lost his balance and fell from the tower; he was killed. Subsequent investigation revealed that the nut securing the bolt that attached the cross-arm piece to the leg of the tower had not been put in place. At the time of the accident, Employee #1 was free climbing and was not using any form of fall protection.

Accident: 170381305 - Injured In Fall While Stepping Off Ladder
At approximately 2:30 p.m. on February 9, 1998, Employee #1, an electrician for H. A. Bowen Electric, was climbing up and down ladders, drilling holes for wiring. He was descending a ladder in a location where there was an accumulation of water on the floor due to a storm. Employee #1 could not see where to place his foot and he did not know of a bolt protruding from the floor. When he stepped off the ladder, his right foot landed on the unguarded bolt. Employee #1 lost his balance and fell, twisting his lower right back and hip area. There were no witnesses to the accident. Prior to his injury, Employee #1 had been performing this same kind of work for many years. The employer has a good and effective Injury and Illness Prevention Program (IIPP).

Accident: 170572390 - Hand Lacerated Against Sheet Metal

Employee #1, a maintenance machinist, was using a 4 1/2 in. vise clamp to hold a piece of 22 gauge sheet metal. He was tightening a bolt when it broke and his left hand struck the side of the sheet metal. Employee #1 sustained a laceration between his first and second fingers that and required six stitches to close.

Accident: 200350320 - Killed When Crushed Inside Drum Of Concrete Truck
At approximately 7:00 a.m. on June 9, 1997, Employee #1, the company owner, was repairing the drum on a concrete truck for a customer. The top half of his body was inside the concrete drum manhole, and he was replacing two rusted bolts inside the drum, when the drum free-wheeled. Employee #1 was crushed and killed. His wife discovered his body at approximately 7:15 a.m. Employee #1 was working alone and there were no witnesses to the accident.

Accident: 200050136 - Killed When Struck By Falling Outrigger
Employee #1 was setting up an outrigger (pivot leg) for a semi-trailer, portable rock crushing unit. He removed two bolts that held the outrigger in the transit position, and pivoted it to the stationary (down) position. The nut on the pivot bolt that was holding the outrigger broke loose and the outrigger fell, striking Employee #1. He was killed.
