Accident: 170205165 - Injured In Fall From Scaffold
At approximately 3:15 p.m. on October 18, 2000, Employee #1, an engineer for Pankow Residential Builders, and a pump operator were using a remote control box to operate a concrete boom pump. Employee #1 was standing on top of a rolling scaffold, assisting laborers in moving the extra length of concrete hose over to the next wall. The boom pump operator was raising the boom slightly to help move the hose when the boom caught the scaffold railing and started to lift it off the ground. Employee #1 jumped off the scaffold, grabbing the top rail of the adjacent wall. The railing broke and he fell backward, away from the wall, striking the back of his head on a concrete slab.

Accident: 170249932 - Back Fractured When Struck By Falling Boom
Employee #1, of Palmer Construction, was using an overhead pumping apparatus owned by Crumpacker Construction to pour concrete into a vertical form. The apparatus came apart and part of the falling boom struck Employee #1's lower back. He suffered fractured vertebrae.

Accident: 14280911 - Injured When Struck By Falling Crane Boom
Employee #1 was part of a crew that was assembling a crane. The crane gantry, which was not fully raised, was in the "floating" position. The crew was standing under the crane boom, on the goose neck of a tractor trailer. When the load line started to raise the erection block lying on the goose neck, the gantry raised and the boom fell. All the workers except Employee #1 were able to jump off the trailer. He was struck by the boom and pinned to the trailer, suffering a broken back and possibly pelvic damage. When the boom was raised, Employee #1 fell from the trailer and struck his head, sustaining a laceration that required six sutures to close. He was hospitalized for his injuries, which were considered life-threatening for several days.

Accident: 14258610 - Electric Shock - Contact With Overhead Line Thru Boom
Three employees were using a crane to load a skid-steer loader onto a trailer. The truck and trailer were parked beneath a 34.5-kilovolt overhead power line (19.9 kilovolts phase to ground). Two of the employees were guiding the loader by hand while the third operated the crane. The crane boom contacted the power line, and the two employees guiding the loader received electric shocks. One of them was electrocuted. The other was hospitalized for his injuries.

Accident: 120126354 - Toes Amputated By Boom Truck Outrigger
At approximately 8:00 a.m. on August 20, 1999, Employee #1, a carpenter employed by Morrison Knudsen Corporation, was working as a swamper unloading soffit materials from a boom truck at a construction site. An outrigger of the boom truck struck and crushed his right foot., amputating three of his toes. Employee #1 was transported by paramedics to Arrowhead Regional Medical Center, where he was hospitalized for two days.
