  • Accident: 170612121 - Carpenter Injured When Struck By Falling Brace

    Employee #1, a carpenter, was helping to move a 9 ft framed wall into place in a single-family house under construction. A brace from an adjoining wall dislodged, striking him just below his waist. Employee #1 suffered a fractured pelvis and a damaged urethra.

    Accident: 800516 - Two Injured In Fall When Scaffold Collapses

    Employees #1 and #2, both plasterers, were working from an approximately 50 ft high, 5 ft long by 7 ft wide single-tower, fabricated frame scaffold. Before finishing for the day, they removed two braces at the bottom to use elsewhere. The following morning they walked up the stairs in the interior of the building and climbed out a window onto the scaffold. The scaffold collapsed with Employees #1 and #2 on it; both workers sustained injuries that required hospitalization.

    Accident: 170564967 - Dies Of Asphyxia When Pinned By Truck Cab

    On February 19, 1998, Employee #1 was replacing the torsion bars for the cab tilt mechanism on a 1993 Peterbilt cab-over truck. At approximately 10:00 p.m., he was found bent over in the right front wheel well, pinned by the closed truck cab's weight. Employee #1 died of asphyxia from external thoracic compression. He apparently did not use the adjustable safety braces supplied by his employer to support the raised cab.

    Accident: 14258685 - Mason Killed In Fall With Scaffold Plank

    Employee #1, a mason, accessed a scaffold at the beginning of his shift. As he was approaching his work area, the plank on which he was walking became displaced from the support bracket. The plank and Employee #1 fell approximately 45 ft to the ground. He was killed.

    Accident: 14444285 - Killed In Fall From Scaffold When End Brackets Fail

    Employee #1, the project superintendent, was on a scaffold when the end brackets and materials hoist collapsed. He fell 101 ft and was killed.

    Accident: 943803 - Killed When Caught Between Dumper And Brace

    At approximately 12:30 p.m. on July 25, 1997, Employee #1 was rounding up stray chickens under a conveyor system when he became caught between a support brace and a steel component of a hydraulic dumper. Employee #1 was killed.

    Accident: 14392872 - One Killed, Two Injured When Broadcast Tower Collapses

    At approximately 2:00 p.m. on March 20, 1997, Employees #1 through #3, were performing maintenance and repair about 200 ft up on a 1,989 ft broadcasting tower. Employees # 1 and #2 were replacing braces on the tower when it suddenly began to collapse. Employee #1 was killed. Employee #2 was thrown into a nearby satellite dish and was hospitalized with multiple injuries. Employee #3, the equipment and parts runner, was thrown approximately 100 yd into an adjacent field. He was treated and released from the hospital. Safety belts and lanyards were in use at the time of the accident.
