  • Accident: 170363162 - Suffers Brain Damage In Fall To Floor

    At approximately 6:00 p.m. on November 19, 1999, Employee #1, a general purpose worker at a large retail grocery, was washing small pieces of equipment in the left-most compartment of a 3three-compartment sink. For some reason, Employee #1 fell and struck the back of his head on the floor. He suffered internal damage to his brain, and was taken to St. Joseph's Hospital in Eureka, CA, where he underwent brain surgery and a lengthy hospitalization.

    Accident: 170045454 - Injured In Fall From Roof

    At approximately 8:05 a.m. on December 8, 1997, Employee #1 was on the 4-in-12 pitched roof of a residence, laying out the air lines for a nail gun. Employee #1 fell 18 ft 11 in. from the edge of the roof onto a brick patio, suffering a fractured skull and potentially permanent brain damage. He was not wearing any form of fall protection.

    Accident: 840249 - Killed When Struck By Vehicle

    Employee #1 was moving a slow moving diesel boom snorkel lift, model #TB37D, across a three-lane highway. He was perpendicular to the line of traffic when a vehicle struck the boom of the equipment and tossed him out of the bucket. He landed on the middle lane of the highway and sustained brain damage, multiple back fractures and internal hemorrhaging. He was killed. Employee #1 was not wearing a belt or harnesses.

    Accident: 200070266 - Killed When Struck By Falling I-Beam

    At approximately 1:00 p.m. on September 8, 1997, Employee #1 arrived at a residential home to pick up excess masonry blocks and cement. He backed his truck, which was equipped with a lifting boom and fork assembly, next to the garage opening. There was no one at the site when Employee #1 arrived. He positioned himself beneath a steel I-beam that spanned the garage opening, and began operating the boom assembly with a remote control box attached to a flexible cable. Using the truck's fork assembly, he picked up a pallet of masonry cement and set it next to a previously placed pallet of masonry blocks. While attempting to retract the forks from under the pallet, he struck the unsecured I-beam and knocked it off the block wall. The beam fell across Employee #1's lower body and knocked him backward. His head struck against the concrete deck and he suffered blunt force trauma to his skull that resulted in cerebral hemorrhaging. Employee #1 was pronounced dead at the scene.

    Accident: 170239990 - Pilot Killed In Helicopter Crash

    Employee #1 was flying a Bell helicopter, model UH-1B, where his coworkers were engaged in logging operations. He was lifting a log that the hooker on the ground had attached to a line secured to the helicopter. The hooker, who was approximately 300 ft from the log, had radio contact with the pilot. The main rotor mast failed, causing the rotor assembly to fly away from the body of the helicopter. The helicopter and pilot then inverted and crashed to the ground. Employee #1 suffered multiple body fractures and severe head trauma, with massive brain damage. He was killed.

    Accident: 170568828 - Dies Of Brain Hemorrhage After Struck On Head

    Employee #1 was performing her duties as a residential treatment worker in a state mental facility when a client became violent and struck her. Employee #1 collapsed and was transported to the hospital, where she later died. The cause of death was a brain hemorrhage, but it was unclear whether it was caused by the client striking Employee #1.

    Accident: 14242150 - Killed When Struck By Piece Of Lumber

    Employee #1 was helping a forklift truck move bundles of steel tubing. A bundle shifted and rolled onto a 4 by 4 piece of wood, which flipped up and struck the employee under the chin, killing him. The coroner reported a laceration to the chin area and a broken jaw; an autopsy revealed that the employee had died of a bilateral contusion of the brain.

    Accident: 200020352 - Injured In Fall From Backhoe Bucket, Later Dies

    Employee #1 was riding in the bucket of a rubber-tired backhoe when he either dropped or fell 8 ft onto the asphalt. He was air-evacuated to the hospital, where he underwent emergency brain surgery the next day and had one-quarter of his brain removed. Employee #1 died on Tuesday, June 3, 1997.

    Accident: 201060589 - Injures Head In Fall From Storage Platform

    Employee #1 was atop a storage platform/mezzanine level at a facilities services warehouse in San Diego, CA. He was sweeping and cleaning up when he fell 8 ft 8 in. off the unguarded platform and struck his head on the concrete floor. Employee #1 was taken to Sharp Hospital, where he underwent surgery to stop hemorrhaging in his brain. He later underwent rehabilitative therapy.

    Accident: 14449904 - Died Of Brain Cancer

    An electric utility employee who had been undergoing chemotherapy for brain cancer was found dead by a coworker at a capped landfill. No workplace environmental factors were found to have contributed to his death.
