  • Accident: 126008879 - Suffers Multiple Fractures In Fall Through Catwalk

    On June 17, 1999, Employee #1, a trained and experienced iron worker, was installing metal floor grating on an approximately 80 ft high catwalk underneath the Coronado Bridge in San Diego, CA. He took a lunch break and was moving along the catwalk when he fell 20 ft through an opening onto a work platform. He suffered four fractured ribs, a fractured right knee, and a fractured left scapula. Employee #1 was not using his safety harness and lanyard at the time of the accident.

    Accident: 896639 - Killed In Fall From Vertical Concrete Form

    At approximately 10:15 a.m. on December 28, 1998, Employee #1 was removing bolts from a vertical concrete form inside a bridge pier. He fell approximately 43 ft to the base of the pier and was killed. Employee #1 was wearing a body harness with one lanyard and he also had a wall chain for a positioning device. At the time of the accident, he was not using his lanyard and the hook on the wall chain had a defective safety latch.

    Accident: 897702 - Killed In Fall When Walkway Collapses

    Employee #1, who had been working on a bridge deck, attempted to descend to a walkway 5 ft below. The walkway gave way, and Employee #1 fell approximately 18 ft onto the highway. He died of head injuries sustained in the fall.

    Accident: 812735 - Killed When Railroad Walkway Collapses

    At 10:50 a.m. on July 10, 1998, Employees #1 through #3, all under 18 years of age, were collecting railbed planks dismantled from an abandoned railroad line. They were working from a pedestrian walkway attached to a bridge. One coworker was driving a truck-type front loader, and Employee #1 and the other coworker were riding on the outside of the cab. When the walkway portion of the bridge collapsed, Employee #1 was asphyxiated under the load and 2 ft of water. The employer was cited for a labor standards violation because of the workers' ages.

    Accident: 170363071 - Injured In Fall From Bridge

    Employee #1 was working on a bridge under construction when he stepped onto a 5 1/2 in. by 30 in. unsecured piece of metal decking and fell from the bridge. He sustained injuries, but was not hospitalized.

    Accident: 201440096 - One Killed, One Injured When Struck By Beam

    Employees #1 and #2 were working on a bridge demolition project when the steel framing suddenly failed and they were struck by a falling beam. #1 was killed and Employee #2 sustained serious injuries.

    Accident: 201401155 - Injured In Fall Through Opening In Bridge

    Employee #1 was cleaning bird droppings from a structural steel bridge when he fell through an unguarded opening in the platform. He sustained injuries that required hospitalization.
