Accident: 658328 - Killed In Fall With Aerial Lift Basket
Employee #1 was operating an Asplundh aerial lift, model LR-50. He was standing in the bucket, with a chain saw and rope, and ascending to cut a tree limb. When he had brought the bucket to a height of approximately 20 ft, the 2 3/4 in. wire rope cable on the lower boom broke, causing the upper boom and bucket to fall. The bucket struck the top of the tool compartment on the left rear side of the truck. Employee #1 was wearing a safety belt and harness, but the sudden impact caused his upper back to slam against the side of the bucket. He suffered internal injuries and died shortly after the accident.

Accident: 170574263 - One Killed, One Injured When Scaffold Board Breaks
Employees #1 and #2 were working on a ten-story condominium building from a two-point suspended scaffold. At approximately the eighth floor, they were lowering the scaffold when the north suspension wire rope broke. Employee #1, who was working at the north end of the scaffold and without fall protection equipment, fell approximately eight floors and was killed. Employee #2, who was working at the south end of the scaffold, was using a safety harness, lanyard, and lifeline. He may have held onto his rope in a manner that did not allow it to completely engage on the lifeline, because he slid down his lifeline and suffered serious injuries when he hit the ground.

Accident: 170132351 - Killed When Hit By Flying Skyline
At approximately 3:00 p.m. on January 15, 1998, Employee #1, a chaser, was fueling a chain saw at the back of the yarder on the landing. As the yarder operator pulled in a turn of two logs, the skyline on the Link-Belt yarder broke. The broken end whipped back approximately 200 ft to the landing, hitting the cab of the yarder and striking the Employee #1, who was still behind it. A shovel operator, who was working close by, heard the skyline break and then noticed Employee #1. He quickly called for help and went to assist Employee #1, who was pronounced dead at the scene from massive head injuries.

Accident: 14392526 - Killed When Diesel Hammer Falls From Crane
At approximately 6:00 p.m. on September 17, 1997, Employee #1, of Premiere, Inc., and other crew members were on the premises of a crane rental business, performing test welding on joints of conductive casing. The employees had been working for 2 to 3 hours making seam welds on the pipes in order to qualify for API certification. They were using a Link Belt pedestal crane with a Demag D46 diesel hammer and leads attached by load line, spreader, and slings. Two Lincoln diesel welders and one electrical arc welder were in use. The welding leads were attached to the casing pipe, which was driven into the ground by the diesel hammer. As the workers were lining up the casing for the next hammering, the load line on the crane snapped, causing the hammer and leads and other rigging equipment to fall. The approximately 34,000 lb hammer pinned Employee #1, killing him. The crane was owned and controlled by the crane rental business.

Accident: 201401080 - Killed When Struck By Falling Casing Hammer
Employee #1 was part of a two-man crew drilling a water well at a residential building site. The drill rig had a drill head and a casing hammer on the mast of the rig. Employee #1 was lowering the drill head when it struck the casing hammer, which was suspended by a wire rope cable. The cable broke and the hammer fell, striking Employee #1 on the head. He was killed.
