Accident: 100101021 - Two Killed When Building Collapsed
Employees #1 and #2 worked for a company that was demolishing a building. The walls had been removed, and the concrete roof was supported by steel columns. The employer then proceeded to cut the columns to weaken the structure so it could be pulled over. Nearly all of the columns had been cut when Employees #1 and #2 went up in a basket attached to the forks of a Forklift to attach a large cable. They had been raised about halfway when the building collapsed on them. Employees #1 and #2 were killed, and the lift operator was injured.

Accident: 813949 - Crushed And Killed When Structure Collapses
At approximately 3:15 p.m. on November 20, 1997, Employee #1 and a coworker were working on the second floor of a three-story concrete/masonry building undergoing complete demolition. Their task was to use hand tools to remove rubber from a room off the central hallway. The rubber was there from previous mechanical demolition efforts using two large backhoes to break out window units and outer walls. The mechanical demolition had ceased over an hour before, and no other hand work was being done that would have affected the stability of structure. The coworker had gone to the first floor to get a tool for breaking up the window frames when the roof, the third-floor masonry ceiling, and the ceiling of the second floor collapsed into the room occupied by Employee #1. He was crushed and killed under tons of masonry, steel, and wood rubble. Two witnesses confirmed that Employee #1 was clearly visible at the window opening and had been talking with one of them when the building collapsed; both stated that Employee #1 had no chance to escape.

Accident: 170572739 - Injured When Crushed By Prefab Building
Employee #1 was performing his normal job duties of delivering prefabricated utility buildings. He was unloading a 10 ft by 10 ft building from a flatbed trailer when his feet slipped from under him and the 1,400 to 1,600 lb structure fell on top of him. Employee #1 suffered fractured ribs and multiple lacerations and scrapes.

Employees #1 and #2 were helping to erect a single-family home. They were framing a gable wall when it collapsed on them. Employees #1 and #2 sustained injuries that required hospitalization.

Accident: 201610037 - Three Killed In Fall When Building Collapses
At 3:30 p.m. on July 31, 1997, Employees #1 through #3 were working on a structure under construction at Portland Airport's parking area. The structure collapsed and they fell approximately 70 ft to the ground and were then struck by steel debris. All three employees were killed.

Accident: 200610103 - Killed By Falling Concrete Block Wall
At approximately 12:39 p.m. on June 26, 1997, Employee #1 was using a dump truck to fill bins with gravel and sand at a concrete manufacturing plant. He obtained material from either a different site or a stockpile within the yard. Employee #1 then drove into a concrete block building that contained four different bins and a conveyor system underneath the grated floor where material was dumped into the appropriate bin. As he approached the exit on the other side of the building, which was 24 ft from the nearest bin, the extended bed on his truck struck the building structure and caused a partial building collapse. A 15 ft long by 9 ft high by 12 in. wide portion of the concrete block wall, weighing 13,260 lb, was torn away from the building and landed on the truck cab, crushing Employee #1 inside. He was killed.

Accident: 171055197 - Injured In Fall From Roof
At approximately 2:30 p.m. on June 2, 1997, Employee #1 was hanging a tarp to contain the overspray from fireproofing that was being installed on the top floor of a building under construction. He was on the roof when the safety cable protecting the roof edge failed. Employee #1 fell 62 ft onto a concrete slab at ground level. He sustained injuries that required hospitalization.

Accident: 999136 - One Killed, Nine Injured In Building Collapse
Employees #1 through #10 and four coworkers, all subcontractors, were working in a three-story wood frame apartment building when winds greater than 46 mph caused the building to collapse. Employee #1, a concrete subcontractor, was crushed and killed. Employees #5, #6, and #8 were trapped between collapsed floor joists for about an hour before they were rescued. Employees #2 through #10 sustained injuries, and Employees #2 through #6 were hospitalized.
