  • Accident: 14373633 - Killed Between Conduit Reel And Bulldozer Arm

    Employee #1 and five coworkers were loading 2 to 9 ft in diameter conduit reels onto the deployment arms of a bulldozer. They had wrapped a chain around one reel and the excavator operator began lifting it when a second reel, which was standing on end next to the first, fell over. Employee #1 became pinned between the conduit reel and the deployment arms of the bulldozer, and was killed. All of the reels were standing on end and, because of space constraints, they were placed on the grade to the highway exit ramp. This meant they were sitting at an angle and leaning toward the employees.

    Accident: 102030343 - Killed In Fall From Bulldozer Bucket

    Employee #1 was killed when he fell out of the bucket of a bulldozer.

    Accident: 101350825 - Killed When Run Over By Backing Bulldozer

    Employee #1 was operating a golf cart to pick up trash in the bark area of the plant, where trucks delivered bark chips for fuel. A bulldozer was used to move the bark, and Employee #1 drove her golf cart onto the bark pile and parked. At the same time, the bulldozer operator was backing down the bark pile to an area where the dozer was normally parked when the driver went on break. The view to the rear of the vehicle was restricted, but it was equipped with a working back-up alarm. The operator turned the bulldozer while he was reversing, apparently without first looking to see if anyone was in the new path of travel, and ran over Employee #1 and her golf cart. She was killed. There were no signs prohibiting unauthorized vehicles or personnel from entering the bark pile area where the bulldozer and large trucks operated. She may have driven to the bark pile to offer the bulldozer operator a ride to the break area.

    Accident: 170122329 - Drowns While Freeing Stuck Bulldozer

    At approximately 10:30 a.m. on July 7, 1999, Employee #1 and a coworker unloaded a bulldozer near the edge of Wolf River in Memphis, TN, to pull another bulldozer out of the water. Two other coworkers were at the top of the bank at the highway. Employee #1 attempted to swim out to the stuck dozer with a cable. He went under, came up once and thrashed about for a few seconds, and then went under again. Employee #1 drowned.

    Accident: 800631 - Killed When Run Over By Bulldozer

    Employee #1, a spotter at a garbage landfill, was directing a garbage truck at the tipping pad. At the same time, a coworker was backing a Caterpillar LPG bulldozer, model #D6M. Apparently, the operator of the bulldozer did not see Employee #1 and backed over him. Employee died of massive trauma. The back-up alarm on the bulldozer was operable, and Employee #1 was wearing a bright yellow raincoat.

    Accident: 201110525 - Two Injured When Tow Hook Fails

    On July 19, 1997, Employees #1 and #2 were riding on the bed of a loaded F-700 flatbed truck at a pipeline project. A Caterpillar D-9 bulldozer was towing the truck up a very steep grade The truck had factory-installed tow hooks, and slings connected the hooks to the dozer. In addition to Employees #1 and #2, the truck was also carrying between 200 and 250, 4 by 6 skids, each of which weighed approximately 20 lb. The dozer had pulled the truck about 250 ft uphill when the hook on the passenger side broke loose, causing the truck to overturn and roll down the hill. Both Employees were thrown off the bed of the truck. They were taken to Henry Mayo Hospital suffering from internal injuries and contusions.

    Accident: 200820066 - Killed Between Bulldozer Grill And Blade

    Employee #1 was engaged in routine maintenance of the cooling system on a Caterpillar D6H bulldozer. He used a truck-mounted 8,000 lb crane to lift the grill off the dozer. Employee #1 was lowering the grill back into place when it became caught on the hydraulic lines that controlled the blade. He set down the pendent control for the crane and positioned himself between the grill and the blade to move the hydraulic lines out of the way. Employee #1 apparently bumped the pendent, causing it to fall. This activated the hoist, pinning Employee #1 between the grill and the blade. He sustained severe internal injuries and was killed.

    Accident: 200540144 - Killed When Run Over By Bulldozer

    On May 23, 1997, Employee #1 was using a Caterpillar D6H track dozer to level a shoulder area. He was last observed backing down an approximately 45 percent slope. He apparently jumped or was thrown from the dozer as it traveled backward over a second slope of between 75 and 100 percent. Employee #1 was struck by the moving bulldozer, pulled under its tracks, and killed. The dozer traveled another 383 ft down the hill before it struck a large tree and stopped.

    Accident: 999110 - Killed By Backing Bulldozer

    Employee #1 and coworkers were constructing a driveway. One coworker was operating a bulldozer while Employee #1 was behind the bulldozer measuring the grade of the earth with a laser. The bulldozer backed over him. He was crushed and killed.
