    Accident: 170100903 - Bus Driver Dies After Suffering Heart Attack

    Employee #1 was ferrying an empty bus back to the terminal when the bus came to rest against a guardrail. A bystander approached it and discovered Employee #1 unconscious. CPR was attempted after contacting Emergency Services, but Employee #1 was pronounced dead at the scene due to a massive heart attack. It appears his death was due to natural causes.

    Accident: 170101893 - Bus Driver Killed When Shot By Passenger

    At approximately 3:10 p.m., Employee #1, a Metro transit bus driver, was headed south on the second trip of his normal route. He was crossing a bridge when he was shot in the abdomen by a passenger. He lost control of the bus, which broke through the bridge barrier, hit the roof of an apartment, and came to rest approximately 20 to 30 feet below the bridge deck. Employee #1 was ejected from the bus and landed on the apartment roof. He was pronounced dead at 3:30 p.m.

    Accident: 170150221 - Mechanic Injured When Pinned Between Two Buses

    On June 29, 1998, Employee #1 a bus mechanic for MTA, was changing the oil at the rear of a bus. A second bus lurched forward when its operator, another MTA employee, stepped on the gas pedal rather than on the brake. Employee #1 sustained a broken right leg.

    Accident: 801175 - Killed When Pinned Under Bus

    Employee #1, a wrecker operator, had come from Missouri to tow back a bus. He used his toe truck to raise the bus and placed two 4-by-4 in. boards under each front tire to allow him to connect his tow truck properly to the bus. Employee #1 then went under the bus to hook up his wrecker to the front axle. The location of the bus had received a lot of rain in the past week and the 4-by-4 blocks began to sink and turn sideways. This caused the bus to slip off the blocks, pinning Employee #1. He died of suffocation.

    Accident: 170134332 - Killed When Backed Over By Bus

    Employee #1 fell out of the back of a bus transporting field laborers on a dirt road. The bus backed over him, and he was crushed and killed.

    Accident: 201570033 - Killed When Crushed Between Bus And Truck

    At approximately 2:13 p.m. on September 30, 1996, Employee #1 and the owner of another company were towing an approximately 40 ft long motor coach to a storage facility. Upon arriving at the storage facility where the bus was to be left, Employee #1 began to disconnect the attachment devices between it and tow truck. He was walking between the front of the bus and the rear of the tow truck when the bus rolled forward and crushed Employee #1. He was killed.

    Accident: 171054356 - Mechanic Killed When Crushed By Falling Bus

    Employee #1 was a mechanic with four years experience in bus repair. He was working on a bus in which the left "tag" wheel tire had experienced a blowout and the resulting debris had damaged the undercarriage. Employee #1 raised up the left rear axle of the bus using a vertical piston, single-point type jack. He did not place a block between the jack's cap and the bus axle structure or crib the axle to prevent it from falling. After jacking up the bus, he removed the "tag" wheel and then was going to replace a broken wheel stud, some lines, and a damaged shock absorber. Employee #1 was apparently removing the brake drum to reach the wheel stud when the bus shifted or moved on the jack and the axle structure fell. When the jack fell, a sway bar landed on the Employee #1's left hand and chest. He was pronounced dead at the scene.

    Accident: 14420269 - Killed When Bus Falls Off Jacks

    Employee #1 had jacked up a charter bus to chain up the tag axle. As he was looking under the bus it rolled forward and fell off the jacks, catching his head between the suspension and the tire(s). Employee #1 was killed.

    Accident: 170632020 - Mechanic Killed When Run Over By School Bus

    At 4:00 p.m. on June 5, 1996, Employee #1, a mechanic, was working under Crown Tandem school bus #5, License #509782, of the Bonita Unified School District, San Dimas, CA. The driver did not realize that Employee #1 was there and after cleaning the bus, he drove over him. Employee #1 was taken to San Dimas Community Hospital, where he was pronounced dead at 5:22 p.m. The employer was cited for serious violations of T8CCR 3203(a)(7) and T8CCR 3314(a).
