  • Accident: 665745 - Killed In Fall From Utility Pole

    Employee #1, who worked for a cable television installation company, was free-climbing a utility pole to install rollers and brackets to run the cable wire. He fell approximately 25 ft from the pole to the ground and was killed. Employee #1 was using gaff climbers and a safety belt and safety strap, but he was not using the safety strap while climbing. A light rain was falling at the time of the accident.

    Accident: 800409 - Electric Shock - Direct Contact With Overhead Line

    A crew of three cable television workers was lashing a cable television cable to a utility pole. One employee was in an aerial lift; another employee was on the ground assisting; and the third employee was about 0.4 kilometers away, directing traffic. The ground worker noticed the employee in the aerial lift slumped over in the bucket, lying on the controls unconscious. The aerial lift bucket was moving in the direction of an overhead power line. The employees on the ground could not safely reach the emergency shutoff. The bucket went into the power line, electrocuting the employee inside.

    Accident: 201401114 - Installing Cable Causes Property Damage

    Employee #1 was installing cable strand wire on a pole when he noticed that the existing strand wire that ran north along the road went around the pole and formed a 90 degree angle. He used a come-along to pull the existing cable because he saw it was only 14 to 15 in. from a 120 V secondary power line. Employee #1 cranked the come-along until he heard a spanking sound. The cable strand had struck the power line, resulting in property damage. No information was provided on employee injuries.

    Accident: 201401122 - Employees Installing Wire Cause Property Damage

    Employees #1 and #2 were installing a new strand wire on utility poles. They were drilling a utility pole when they struck a 7,200 V riser and blew the fuse. No information was provided on employee injuries.

    Accident: 14279996 - Cable Repairman Injured In Fall From Ladder

    Employee #1, a cable repairman, was making repairs when he fell approximately 15 feet from a ladder. He was hospitalized.
