  • Accident: 170205827 - Two Injured When Struck By Loaded Cart

    At approximately 7:30 a.m. on July 23, 1999, Employees #1 and #2, both iron workers, were on the third floor waiting for a load of "stirrups" (re-bar sections) to be landed. The bundle came through a wall opening, and they landed the load onto a cart. They repositioned the stirrups in the cart and, within a minute, one end of the floor dropped to the second floor, at an angle. The cart with the stirrups pinned Employee #1 against the wall, and he suffered a fractured right cheek and facial lacerations. Employee #2 was lodged next to the wall opening and suffered a contusion of his right thigh. They were taken to San Francisco General Hospital.

    Accident: 120323878 - Killed When Forklift Collides With Electric Cart

    At approximately 12:40 p.m. on April 7, 1999, Employee #1, a welder/mechanic employed by Rialto Concrete Products, Inc., was operating an electric vehicle in the pipe storage yard. It was struck by a large forklift loaded with five sections of 8 ft by 18 in. concrete pipe, which was traveling in the opposite direction. Employee #1 was killed.

    Accident: 665737 - Killed When Cart Becomes Caught Under Trailer

    At approximately 2:00 p.m., Employee #1, while driving an electric-powered cargo and personnel cart to the area around the trash dumpster. At approximately 2:54 p.m., two coworkers discovered Employee #1's body in the cart. He had been killed. Employee #1 had apparently driven under the front of a disconnected tractor/trailer next to the dumpster, and had become caught and crushed.

    Accident: 119957801 - Injured By Backing Industrial Truck Cart

    At approximately 6:10 p.m. on February 2, 1999, Employee #1 was seriously injured when an industrial truck utility cart was backed into him and pinned him against a rail car. He sustained abrasions and contusions, and was hospitalized for three days.

    Accident: 170074256 - Killed When Caught Between Carrier And Rail

    Employee #1 was moving a carrier into the work area when he became trapped between the carrier and the rail. He was killed.

    Accident: 170387815 - Killed When Struck By Falling Metal Cart

    Employee #1 was at an outside door using a lift truck to retrieve approximately 46 in. long by 57 in. wide by 45 in. tall metal carts stacked on top of each other in racks. Each rack had four 8 in. metal wheels, and the top of the racks had 5 in. high metal toeboards on three sides. The racks were 56 in. behind a semi-trailer and angled with the right rear wheel of the bottom rack on top of a concave drain grate. Employee #1 was between the stacked carts and the semi-trailer, with the open sides facing the trailer, when he pulled on the bottom rack to straighten and remove a cart. The top rack rolled off of the bottom rack, pinning Employee #1 against the semi-trailer. He was killed. Each cart weighed approximately 500 lb.

    Accident: 170572382 - Leg Fractured When Struck By Falling Buggy

    Employee #1 was performing his usual task of moving buggies on a concrete dock when he lost control of the buggy and fell 4 ft to the ground. The cart fell on top of him, fracturing his leg above the ankle.

    Accident: 170616023 - Injured When Crushed Between Push Cart And Wall

    At approximately 7:45 p.m. on December 8, 1997, Employee #1, of Mercy Hospital, was pushing a food cart down a ramp when she lost control of it. As she tried to stop the cart, she was crushed between it and a wall. Employee #1 suffered a fractured ankle and a lacerated ear, for which she was hospitalized.

    Accident: 125892356 - Fingertip Amputated Between Cargo Dollies

    At approximately 1:18 a.m. on December 5, 1997, Employee #1, a ramp worker for United Parcel Service, was connecting two cargo dollies on the UPS ramp at the Ontario (CA) Airport when the tip of her left middle finger was amputated. She was taken by private automobile to the Chino Valley Hospital, where she was treated and released.
