  • Accident: 200621688 - Run Over By Forklift

    A carnival worker was climbing into a metal basket while it was being moved by a boom attached to the front end of an R80 Caterpillar forklift truck. The employee lost balance, and his foot got caught under the left front wheel of the truck. He was pulled under the forklift, and both wheels ran over him before the driver could stop it. The carnival worker died of his injuries.

    Accident: 801217 - Killed When Struck And Dragged By Forklift Truck

    Employee #1 asked another crew leader about the location of a jack and, after finishing the conversation, began to walk back across an aisleway. A coworker driving an Ottawa yard truck pulling a 12 ft wide load struck him from behind. Employee #1 was dragged between 60 and 70 ft by the strap on his coveralls, which caught on the tie rod of the truck. He died instantly of a broken neck.

    Accident: 813386 - Killed When Caught In Mixer

    On October 16, 2000, Employee #1, age 18 year, was cleaning a mixer. He was reaching inside when the mixer engaged and he was drawn in. Employee #1 was killed. The mixer was not locked out, and a device had been used to override the two-hand controls.

    Accident: 170009351 - Injured In Fall From Roof

    At approximately 7:30 a.m. on September 9, 2002, Employee #1 climbed to the roof of a building, walked west across installed decking, donned his fall protection harness, hooked up his Guardian Fall Protection Beamer 2000, and walked the joist out to his work area where he was stick-welding bridging on the roof. At about 10:00 a.m., when he was about one-third finished, he hooked up his Beamer at the center seat. He walked east along the joist to the water cooler for a break. His Beamer became caught on something, pulling him backward and off his feet. Employee #1 fell sideways and his lanyard stopped his fall, partially activating the lanyard deceleration device. He bounced back up and when he descended, the Beamer came free from the beam. Employee #1 fell between 20 and 22 ft onto dirt, fracturing his back and ankle.

    Accident: 14228621 - Arm Amputated In Fiber Rewind Roller

    Employee #1 was reattaching a loose piece of fiber that had come off a moving rewind roll when his glove became caught in the machine. His arm was pulled into the roll and amputated.

    Accident: 123304750 - Hand Injured When Caught In Conveyor Belt

    Employee #1, the lead person on the shift, was reaching into a moving conveyor belt to clear a jam when his hand was pulled into the belt. He sustained injuries that required hospitalization.

    Accident: 14216220 - Killed In Fall Through Skylight

    On May 30, 2000, Employee #1 was washing down an aeration basin to prepare it for a curtain wall installation on June 5, 2000. Employee #1 was walking backward while pulling a 1 1/2 in. uncharged water hose across grade level, where there were four 46 1/4 in. by 46 1/4 in. by 15 in. high skylight/hatchways. A coupling on the water hose became lodged in a graded walkway and, when Employee # 1 tried pulling it free, he tripped over the curbing on one of the grade-level skylight/hatchways. He went through the cover of the skylight/hatchway and fell approximately 19 ft into the pump house basement. Employee #1 was killed.

    Accident: 113549968 - Fingers Injured In Press

    Employee #1 was dislodging a piece of material from a press when he and a coworker accidentally activated the press. His index finger and thumb became caught, and he sustained serious injuries.

    Accident: 125890723 - Fingers Lacerated In Hopper/Picker

    At approximately 10:00 a.m. on April 19, 2000, Employee #1 was unclogging a hopper/picker. The machine caught his left hand, severely lacerating three of his fingers. Employee #1 was transported to Arrowhead Regional Medical Center, where he underwent surgery and was hospitalized for two to three days.
