  • Accident: 170572176 - Hand Injured In Sprocket Drive Of Packing Machine

    Employee #1, age 18, was performing preventive maintenance on a packing machine, cleaning the main chain and sprocket drive, when his right hand became caught in the sprocket drive. The first joint of his index finger was amputated, his third and fourth fingers were fractured, and the top of his hand was lacerated. The machine was running and had not been locked out when Employee #1 started work on it.

    Accident: 119901387 - Finger Injured When Caught In Chain And Sprocket

    At approximately 8:30 a.m. on December 1, 1999, Employee #1 was helping two other maintenance mechanics repair an inclined conveyor. Employee #1's left hand was on the chain and sprocket. When the conveyor started, catching his thumb and three fingers. His left thumb was partially amputated, his index and middle fingers sustained multiple fractures, and his ring finger was lacerated. He was hospitalized for three days and underwent three surgeries on his hand. The conveyor was not de-energized or locked out prior to it being repaired.

    Employee #1, age 19 and part of a sanitation crew, became caught between a horizontal chain and sprocket. He was killed.

    Accident: 120125976 - Fingers Amputated In Chain And Sprocket

    At approximately 2:10 p.m. on May 22, 1999, Employee #1, an equipment lubricator employed by the Mission Foods Corporation, was removing excess lubricants from a rotating chain and sprocket assembly. The first digits of the index and middle fingers on his right hand were amputated. Employee #1 was transported by paramedics to the Chino Valley Medical Center, where he was treated and released.

    Accident: 125408005 - Killed When Crushed By Falling Tank

    Employee #1 was hoisting a chiller tank when the rigging chain broke. The tank fell and rolled, pinning Employee #1 between the tank and a concrete block wall. He was crushed to death.

    Accident: 170616395 - Hand Injured In Chain And Sprocket Drive

    At approximately 7:30 p.m. on July 29, 1998, Employee #1, of Grimmway Farms, Inc., was applying grease to a moving chain when his right hand became caught in the chain and sprocket drive. He suffered a partial amputation of four fingers and was taken to a local hospital in Bakersfield, CA, before being transferred and admitted to Davis Medical Center in San Francisco, CA.

    Accident: 170616122 - Fingers Caught In Chain And Sprocket Drive

    At approximately 3:00 p.m. on May 21, 1998, Employee #1 was adding water and chemicals into a nurse tank on a Royal bean spray rig when he became dizzy. He was stepping down from the work platform when his right hand became caught in the sprayer's moving chain and sprocket drive, partially amputating two fingers. He was admitted to Mercy Hospital in Bakersfield, CA, for treatment.

    Accident: 170361968 - Foot Injured In Mill Cant

    Employee #1 had a short cant that became hung up and was causing the mill cant chain to jam. The chain is governed by a HOLD-DOWN button, and Employee #1 used a stick to wedge the button into the 'on' position. He then got on the running chain to pry out the short cant. When the short cant came free, Employee #1 lost his balance and another cant trapped his foot against the roll case, which was receiving the cants from the cant chain. His foot sustained injuries, for which he was hospitalized. Employee #1 was an experienced mill cants button operator. This accident resulted from his overriding the HOLD-DOWN button and from his mounting the machine as it was running, instead of locking it out.

    Accident: 170204093 - Hand Injured In Chain And Sprocket Assembly

    At approximately 7:45 a.m. on November 24, 1997, Employee #1, a feed pack operator at Crown Cork Seal, was working alone, picking up scrap metal. He was attempting to fix a jam in press #5 when his gloved left hand became caught by the rotating sprocket and was pulled into the chain and sprocket assembly. Four fingers on his left hand were either lacerated or amputated. No accident-related violations were found, and no citations were issued.

    Accident: 170127450 - Fingers Amputated In Chain Drive

    Employee #1 was piping chitterlings when the glove on her right hand became caught by the chain drive, amputating her index, middle, and ring fingers. Neither the sheet metal guard nor the guard for the chain drive were in place at the time of the accident.

    Employee #1 was struck on the head by a chain saw when the aerial lift he was in suddenly dropped. He sustained a severe laceration on his head and was killed.

    Accident: 200020501 - Finger Injured When Caught In Wrapping Machine

    Employee #1 was bending down to pick up muffins from the floor when her left middle finger became caught in the drive chain of the Doughboy feeder wrapping machine. She was transported to St. Joseph's Hospital in Paterson, NJ.

    Accident: 200540219 -- Report ID: 0317500 -- Event Date: 10/23/1997

    At approximately 2:00 p.m. on October 23, 1997, Employee #1 was using a chain saw to trim a tree limb when he accidentally cut his safety line. He fell 30 ft from the tree to the ground, and was killed.
