  • Accident: 896589 - Injured When Neck Impaled On Gear Shift

    Employee #1 was using an over-the-road tractor truck to pull a house to a new location on a trailer-type assembly. The chimney, which had been braced and supported, became dislodged from the house and fell onto the cab of the truck. Employee #1's neck was impaled on the gear shift, and he was hospitalized for treatment of his injuries.

    Accident: 201401148 - Killed When Struck By Debris From Falling Chimney

    On September 24, 1997, Employee #1 and a coworker were tearing down an unsupported 20 ft high chimney at a job site in Calhoun County. His coworker was standing at the entrance to the building and Employee #1 was starting to pry off bricks from the bottom of the chimney when the entire structure began to come down. Employee #1 ran but was struck on the head by falling bricks and debris. He was pronounced dead at the scene.

    Accident: 758870 - Killed In 350 Ft Fall From Scaffold

    Employee #1, a laborer, was in a chimney stack on an elevated suspended scaffold approximately 350 ft high, helping bricklayers lay brick inside the liner of the chimney. As Employee #1 was attempting to open trap doors for the hoistway area on the suspended scaffold, he fell through the hoisting area approximately 350 ft to a metal hopper surface at the base of the chimney. He was killed. Employee #1 was not wearing fall protection while on the scaffold or while attempting to open the hoistway doors on the scaffold. The hoistway area was not guarded.

    Accident: 657916 - Dies In Fall From Scaffold Or Roof

    Employee #1 fell from a scaffold or roof while working on the chimney. He landed on pressure treated porch joists that were about 16 ft 10 in. from the ground. He died.

    Accident: 573733 - Dies In Fall From Roof While Repairing Chimney

    Employee #1 was on the roof of a house repairing damage to the chimney and flashing. While Employee #1 was applying a silicone sealant to the flashing, the chimney broke loose and he fell to the ground, striking his head on the neighboring house approximately 5 feet above the ground. Several of the stones from the chimney struck the employee and the adjacent house. Employee #1 sustained several fractures to the head and died.
