  • Accident: 897678 - Killed When Caught In Wood Chipper

    Employee #1, age 20, was jumping up and down on a conveyor line in an attempt to unjam scrap material feeding into the chipper while the conveyor was running. He fell onto the conveyor and was carried into the chipper. Employee #1 was killed.

    Accident: 1003631 - Fingers Amputated In Chipper

    Employee #1 was feeding material into a gasoline-powered brush chipper when the material became caught on her left glove. The feed rolls on the equipment pulled her left hand and arm into the wood chipper, amputating her fingers and one-half of her thumb.

    Accident: 201700101 - Killed When Crushed Inside Chipper

    At approximately 9:00 a.m. on August 7, 1997, Employee #1, of the Boardman Chip Co., Inc., was performing maintenance on a ring debarker machine. He and a coworker were inside the debarker feed roll sprocket area to troubleshoot and to inspect the cause of equipment failure. None of the energy sources was locked or tagged out, nor had safety pins been installed to prevent unintended movement. For some reason, the electrical power shut down. The air released, causing an upper and a lower sprocket to come together, pinning Employee #1. He was transported to the hospital, where he died.

    Accident: 170888366 - Fractures Leg In Fall While Clearing Wood Chipper

    Employee #1 was learning how to clear a jam from a wood chipper at a wood byproducts plant. He was standing atop the approximately 3 ft high shaft guard, trying to clear the machine when he fell. He sustained a compound fracture of his right leg. Employee #1 had worked at the plant for approximately 6 months, and at the wood chipper for about 1 1/2 weeks. He was not using extension tools to try to clear the machine.

    Accident: 200670636 - Killed When Caught In Wood Chipper

    Employee #1 was grinding tree branches with a self-feeding wood chipper. He was feeding material into the hopper when he apparently attempted to dislodge or push through jammed material with his foot or hand. Employee #1 was caught by the in-feed wheels and pulled into the chipper blade. He was killed.

    Accident: 951707 - Killed When Caught In Wood Chipper

    At approximately 5:00 p.m. on April 23, 1997, Employee #1 was feeding pruned tree limbs into a Morbark E-Z Bever wood chipper. He made contact with the feeder drum, located 63 in. from where he was standing on the ground at the infeed tray, and was grabbed by the feed wheel. He was then drawn into the chipper drum located inside the machine, and chopped to death by the chipper blades. The employee's remains were deposited into the wood chip receiver truck through the discharge spout of the chipper. There is a reversing safety bar for the feeder drum, located at the mouth of the infeed. It is bail shaped, and goes down both sides and across the top of the feeder.
