  • Accident: 813386 - Killed When Caught In Mixer

    On October 16, 2000, Employee #1, age 18 year, was cleaning a mixer. He was reaching inside when the mixer engaged and he was drawn in. Employee #1 was killed. The mixer was not locked out, and a device had been used to override the two-hand controls.

    Accident: 14222541 - Dies After Suffering Heat Stroke And Drowns

    At approximately 3:00 p.m. on June 19, 2000, Employee #1, age 16, was cleaning up trash in a ditch in preparation for cutting the grass. He was found dead, lying face down in the ditch, which was filled with approximately 8 in. of water. Employee #1 was last seen alive at approximately 2:00 p.m. The autopsy report stated the cause of death as environmental heat exposure followed by asphyxia secondary to drowning.

    Accident: 170243778 - Found Dead On Floor Of Work Place

    At 5:30 a.m. on May 15, 2000, Employee #1 was found lying on the floor next to a floor buffer he had been operating. His supervisor noted that he was not breathing and called for assistance. CPR was started immediately and was continued until paramedics arrived approximately ten minutes later. Employee #1's death was determined to be from natural causes.

    Accident: 784710 - Injured In Fall While Cleaning Tanker Trailer

    At 10:35 a.m. on May 9, 2000, Employee #1, age 19, was using a high pressure sprayer to clean the inside of a tanker trailer. He either was standing on platforms near the top of the trailer or was on the fixed ladder on the side of the trailer. Employee #1 apparently became overcome by the smell of the chicken blood being hauled in the trailer and fell from the trailer. He sustained injuries that required hospitalization.

    Accident: 960294 - Breaks Leg In Fall On Wet Floor

    Employee #1, a deli clerk, was returning from the warehouse to her department. She cut through the meat department in the supermarket while the floor was being cleaned and was slippery. Employee #1 slipped and fell, breaking her left leg. The wet floor was not marked in any manner.

    Accident: 14258842 - Injured When Struck By Flying Door

    Employee #1 was cleaning up in front of a mercury retort unit. A fire inside the unit increased the interior pressure and the door blew off its hinges, striking Employee #1. He sustained injuries that required hospitalization. Heavy aliphatic hydrocarbons were found in two metal vessels within the retort unit.
