Accident: 170205140 - Leg Injured When Run Over By Forklift
At approximately 2:00 p.m. on August 22, 2000, Employee #1, a laborer, was working as a flagman, controlling traffic and placing cones for pedestrians and cars. The owner of the company, who was operating a Gradeall forklift, model 544D, put it into reverse, and Employee #1 moved the orange cones out of the way of the machine. As the owner again moved forward, Employee #1 brought the cones forward to their original location. The Gradeall has hydraulically power steering, with the rear wheels steering a 90-degree pirouette around either front tire; the outside turning radius is less than chassis length. As the forklift went forward and Employee #1 was placing the cones, the rear wheel of the vehicle struck his right leg and rolled over it, stopping while his leg was partially under the wheel. Coworkers placed some lumber under the wheel and backed up he machine to free Employee #1's leg. No accident-related violations were found.

Accident: 170363279 - Suffers Acid Burns On Eye
Employee #1, an associate in the garden department at Home Depot store #5211, and a coworker were removing lawn mowers from packing crates. Employee #1 was using the forks of a lift truck to take apart a crate. As he approached the crate, the truck struck and wedged a1 gal jug of muriatic acid (swimming pool grade) against a wooden pallet. The acid was squeezed out of the bottle, splashing onto Employee #1's face and right arm. He suffered first-degree burns to his right eye and on other exposed areas. Employee #1 was transported to Redding Medical Center, where he was treated and released.

Accident: 119897197 - Injured When Crushed By Beam
At approximately 12:08 p.m. on April 4, 2000, Employee #1 was moving an I-beam when it was struck on the opposite end by another I-beam as it exited the automatic saw. Employee #1's beam went into his abdomen, and it lifted and pushed him into a shelf rack and up against stacked hangers. Employee #1 became caught between the rack and the beam; he sustained injuries that required hospitalization.

Accident: 170572994 - Killed When Struck By Falling Tree
Employee #1 and a coworker were clearing trees, brush, and beaver dams along a creek. Employee #1 cut a 65 ft tall, double-trunk tree, which did not immediately fall. He had moved approximately 45 ft away to adjust his chain when the tree fell over and struck him. Employee #1 was killed.

Accident: 122090418 - Fingers Amputated In Press
Employee #1 was operating a full-revolution Niagara press when he inadvertently amputated two of his fingers. The press was unguarded and tripped by a two-hand control. The trips were right up to the press and did not have the required distance. In addition, the controls did not require concurrent pressure.

Accident: 170272041 - Killed When Struck By Excavator
Employee #1, a laborer at a trash refuse company, was struck by the right rear superstructure of an excavator as it was making a turn. He was killed.
