Employee #1 asked another crew leader about the location of a jack and, after finishing the conversation, began to walk back across an aisleway. A coworker driving an Ottawa yard truck pulling a 12 ft wide load struck him from behind. Employee #1 was dragged between 60 and 70 ft by the strap on his coveralls, which caught on the tie rod of the truck. He died instantly of a broken neck.

At approximately 2:00 p.m. on May 15, 2000, Employee #1, an adhesive compound mixer for W. F. Taylor, Inc., was working at an adhesive manufacturing facility. He was splashed with a hot chemical mixture he was preparing and sustained second-degree burns to his right arm and hand. Employee #1 was transported to Arrowhead Regional Medical Center, where he was hospitalized for eight days.

Accident: 14222301 - Killed When Caught In Auger
At approximately 8:20 a.m. on March 13, 2000, Employee #1 and coworkers were digging post holes to install a security fence on his employer's property. They were using a Massey-Ferguson #231 tractor with an attached post hole digger (auger). Employee #1 was walking around to apply pressure to the guide on the auger when his left pant leg and shirt became entangled in the auger's driver shaft. His left leg was twisted and amputated below the knee. Employee #1 became caught in and was thrashed around the auger numerous times, his head apparently striking the drive line of the auger. He was killed. The auger did not have any guards or shields to protect workers from becoming caught.

Accident: 14228209 - Burn - Electrical Fault In Electrical Box
An employee was removing conductors from a conduit. He was pulling them through a transformer switch box. A bare equipment grounding conductor touched an energized circuit conductor inside the box, causing an electrical fault. The ensuing electric arc started a fire inside the box and ignited the employee's clothing. The employee sustained second- and third-degree burns to his face, arms, hands, and stomach. He was hospitalized for his injuries.

An employee was replacing an electric service. He was using a metal utility knife to cut through insulation on a deenergized 2400-volt conductor. The knife contacted an energized part and a grounded surface at the same time, causing an electrical fault. The employee received first- and second-degree burns over 60 percent of his body from the ensuing electric arc. He died from his injuries.
