  • Accident: 693374 - Injured In Fall While Cleaning, Later Dies

    At approximately 4:30 a.m. on October 12, 1999, Employee #1, a member of a sanitation crew, was spraying down the machinery and floor of the pet food area. Approximately 10 minutes later, coworkers found him lying on the floor, stating he did not remember what had happened. He suffered a fractured skull, a broken collarbone, and fractured ribs on his right side. Employee #1 was transported to the hospital, where he died on October 16, 1999, as a result of his injuries.

    Accident: 170380364 - Wrist Shattered In Fall From Ladder

    Employee #1, an iron worker at a construction site, was assembling a sheet metal aircraft hangar. He was using a 24 ft Alaco industrial aluminum extension ladder that was clamped to the building to descend from the roof. As Employee #1 got on the ladder, he moved it vertically, causing the latches to release and the ladder to collapse. He fell approximately 19 ft 8 in. to the ground, suffering a shattered right wrist, two fractured ribs, and a broken collarbone. His wrist required surgical pins to facilitate healing.

    Accident: 125892513 - Fractures Collarbone In Fall From Hangar Roof

    At approximately 7:39 a.m. on August 7, 1998, Employee #1 was reroofing an aircraft hangar at the Chino (CA) Airport when he fell 19 ft 9 in. He suffered a fractured clavicle and lacerated right forearm. Employee #1 was transported by helicopter to Loma Linda University Medical Center, where he was hospitalized.

    Accident: 170132377 - Injured In Fall From Roof

    At approximately 6:00 a.m. on January 29, 1998, Employee #1 was steam cleaning a kitchen ventilation hood at Guild's Lake Inn Restaurant in Portland. He placed a ladder on the outside of the one-story building and climbed to the roof carrying a 1.5 gallon sprayer, two-thirds full of caustic soda. After spraying the hood, he returned to the ground, got the steam cleaning hose, and returned to the roof. At some point Employee #1 fell; he was found on the ground by a coworker, with blood coming from his head and ear. Employee #1 sustained a fractured skull and clavicle and was transported to the hospital.

    Accident: 170132344 - Injured In Fall From Roof

    At 9:50 a.m. on December 22, 1997, Employee #1 was rolling out #4 wire. He tripped and stumbled about 10 to 15 ft onto another roof, and then fell approximately 22 ft to the gravel roadway. Employee #1 sustained a concussion, fractured ribs, and a fractured collarbone. He was transported to Providence Hospital in Medford, Oregon.
