At approximately 1:00 p.m. on March 23, 2000, Employee #1, a heavy equipment operator, was working at Badlands Sanitary Landfill. The scraper he was operating collided head-on with a motor grader, and he suffered serious neck and back strain injuries. Paramedics responded to the scene, and Employee #1 was air-lifted to the Riverside County Regional Medical Facility, where he was evaluated, treated, and hospitalized for more than 24 hours. On November 10, 1999, Employee #1 was driving a Peterbilt side-loading garbage truck south on Highway 99 when he failed to stop in time and rear-ended a Mazda sedan. The truck veered right and Employee #1 corrected the vehicle's direction by turning left. The centrifugal force pitched him out of the truck and he landed on the pavement. He sustained massive head injuries and died at the hospital the following day. Employee #1 was in the right-hand driving position; he was not wearing a seat belt.

It was struck by a large forklift loaded with five sections of 8 ft by 18 in. concrete pipe, which was traveling in the opposite direction. Employee #1 was killed.

At 8:00 a.m. on March 30, 1999, Employee #1 was driving westbound on Route 8 from Olympia, WA, en route to a customer. A light dusting of snow covered the road. At milepost 11, Employee #1's vehicle was seen leaving the road and crashing into some trees. He suffered hip, leg, and facial fractures, and internal bleeding. Employee #1 was transported to Harborview Hospital, where he died on April 13, 1999.
