Accident: 952002 - Killed In Fall From Edge Of Roof
Employee #1 was repairing the gutters at the edge of an the roof of a three-story building. His foot was braced against an ice cleat on the metal roof when the cleat broke and he fell approximately 70 ft to the sidewalk. Employee #1 died of massive trauma.

Accident: 119948016 - Injured In Fall While Framing House
At approximately 7:00 a.m. on December 1, 2000, Employee #1 was framing a residence when he fell 9 ft 6 in., sustaining a fractured right arm and lacerated liver. Employee #1 was transported by paramedics to JFK Memorial Hospital, where he was hospitalized for five days.

Accident: 170205173 - Injured When Struck By Falling Beam
At approximately 9:45 a.m. on November 9, 2000, employees of the Hathaway-Dinwiddie Construction Company were stripping the deck of 2 by 4 falsework when part of the deck popped loose and knocked an 8 in. by 8 in. by 7 ft beam toward Employee #1. He was standing directly in front of the beam and could not get away in time. The beam fell, striking his head, face, nose, left hand, and knee. Employee #1 was stunned and felt slightly dizzy, but did not lose consciousness. No accident-related violations were found in connection with this incident.

Accident: 14225759 - Killed When Head Crushed Under Trailer Ramp
At approximately 2:00 p.m. on November 2, 2000, Employee #1 was helping his employer to unload a Bobcat loader from a lowboy trailer. As the employer was unchaining the Bobcat, he asked Employee #1 to drop the ramps, which he did. The chain on the Bobcat became caught, and the employer concentrated on releasing it and did not notice Employee #1. Another coworker was standing to the right of the trailer watching the employer free the Bobcat. The employer then mounted the Bobcat and said that he was ready to back it off. He looked behind him and did not see Employee #1, so he assumed that he had gone to the office. As the employer began backing up and the rear wheels touched the top of the ramps, the coworker standing to the side screamed for him to stop. Employee #1 was found face down with his head under the right ramp and his feet under the left ramp. He suffered massive multiple fractures and internal injuries to his head, and was killed. There was no indication as to why Employee #1 was in that location.

Accident: 812685 - One Killed, One Injured When Traffic Light Unit Falls
Employees #1 and #2 were on an eight-person crew that was installing a traffic signal light with a cross-arm extending over a traffic island. The unit was lifted into position using the boom of a digger truck, and four employees were holding tag guide lines at the outer end of the cross-arm as it was raised. The load line broke, and the traffic light unit fell, striking Employees #1 and #2. Employee #1 was killed and Employee #2 sustained injuries that required hospitalization.
