Accident: 656173 - Knee Bruised In Same-Level Fall
Employee #1 was assigned to adjust some stock at the top of a pallet, which required that he be raised from a guarded work platform. He had on a full body harness and was attached by a lanyard and chain to the top rail of the guardrail system. This allowed Employee #1 to exit the work platform and climb onto the pallet to adjust the stock. He was standing on the pallet when a piece of wood broke and he fell through up to his crotch. Employee #1 sustained bruises to his knee.

Accident: 126078518 - Leg Injured In Fall Between Truck And Dock
At approximately 10:30 p.m. on September 20, 1999, Employee #1 was off loading merchandise from a truck onto the receiving dock of a Target store when his right leg slipped into the 6 to 9 in. wide gap between the truck and the dock. He suffered a sprain and contusions to his upper leg. A coworker helped him out of the gap and paramedics took Employee #1 to Alhambra Medical Hospital for treatment. Subsequent investigation revealed that no dock plate or ramp had been used to span the gap between the truck and loading dock. The employer was cited for a serious violation of T8CCR 3337(b).

Accident: 898692 - One Killed, Two Injured When Struck By Car
Employees #1 through #3 were working in a construction zone. Employees #2 and #3 were installing a replacement concrete curb in the zone, which was marked off by four 18 in. orange traffic cones set on a white line that represented a residential parking area. They were struck by an automobile that was passing on the passenger's side of other vehicles, which were slowed by a 32-ton ready mix concrete truck that was impeding the westbound flow of traffic on that two-lane street. The vehicle then struck Employee #1, who was setting out more orange cones at the rear of the concrete truck. Fairfax County paramedics were unable to obtain any vital signs from Employee #1, and he was pronounced dead at the scene. Employees #2 and #3 were transported to the Fairfax Hospital Shock/Trauma unit. Employee #2 was treated for minor lacerations and bruising and released; Employee #3 was taken to surgery in critical condition.

Accident: 170205827 - Two Employees Injured When Struck By Loaded Cart
At approximately 7:30 a.m. on July 23, 1999, Employees #1 and #2, both iron workers, were on the third floor waiting for a load of "stirrups" (re-bar sections) to be landed. The bundle came through a wall opening, and they landed the load onto a cart. They repositioned the stirrups in the cart and, within a minute, one end of the floor dropped to the second floor, at an angle. The cart with the stirrups pinned Employee #1 against the wall, and he suffered a fractured right cheek and facial lacerations. Employee #2 was lodged next to the wall opening and suffered a contusion of his right thigh. They were taken to San Francisco General Hospital.
