Accident: 200621688 - Run Over By Forklift
A carnival worker was climbing into a metal basket while it was being moved by a boom attached to the front end of an R80 Caterpillar forklift truck. The employee lost balance, and his foot got caught under the left front wheel of the truck. He was pulled under the forklift, and both wheels ran over him before the driver could stop it. The carnival worker died of his injuries.

Accident: 200270833 - Crushed While Greasing Amusement Ride
Three employees were greasing a carnival ride. Two of them were under the ride. The third employee was standing on a platform above the ride. One of the employees under the ride (employee #1) instructed the worker next to him to hold the end of the grease gun while the first employee pumped the gun. Employee #1 instructed the employee on the platform to begin turning the upper rim of the ride, which sat at a 15-degree angle during operation. As the employee on the platform began to turn the rim, the heaviest part of the ride rotated down hill. Its momentum caused the ride to continue turning. The second employee under the ride dove to the ground out of the way of the rotating supporting structure. Unfortunately, employee #1 was crushed between the ride's rotating turret and the stationary trailer frame. He died of his injuries.

Accident: 170401574 - Crushed Between Backhoe Bucket And Outrigger
Employee #1 had gotten off the backhoe he was operating and left the motor running. He was killed when he was struck by the backhoe and crushed between the bucket and outrigger.

Accident: 170019343 - Killed When Tractor Overturns During Loading
Employee #1, the owner-operator of a farm tractor with a rake attachment, was attempting to load the vehicle onto a flatbed truck. He was driving up two metal ramps when the tractor tipped over and fell off the ramps, trapping him underneath it. Employee #1 was crushed and killed.

Accident: 125965731 - Toe Crushed When Run Over, Later Amputated
At approximately 11:15 p.m. on November 8, 2000, Employee #1 was unloading pallets and containers from a Boeing 767 when her big toe was run over by an air cargo pallet. She was transported to Arrowhead Medical Center in Colton, where her toe had to be surgically amputated after efforts to save it failed. She was hospitalized between two and three weeks.

Accident: 116211962 - Crushed And Killed Under Truck
Employee #1 was working on the starter of a Freightliner truck cab, which was supported by a wooden ramp under the front wheel on the driver's side. The truck wheels were not chocked, and the truck did not have brakes. While Employee #1 was underneath the truck, it rolled forward and pinned him in place. He was crushed and killed.
