  • Accident: 119947307 - Iron Worker Injured In Fall While Installing Decking

    At approximately 8:30 a.m. on January 4, 2000, Employee #1, a iron worker for the McIntyre Company, Pomona, CA, was installing metal decking for the fine arts building at the University of California, Riverside. He fell 14 ft 7 in. and sustained multiple facial injuries and a fractured right wrist. Employee #1 was transported by paramedics to the Riverside Community Hospital in Riverside, where he was hospitalized for 26 hours.

    Accident: 656108 - Injured In Fall From Structural Steel

    Employee #1 was welding some decking onto structural steel when the welding cables fell between the supports. This caused Employee #1 to lose his balance, and he fell 15 ft to the ground.

    Accident: 100351808 - Killed In Fall From Floor Decking

    Employee #1, who was engaged in steel erection, fell 25 ft 9 in. from floor decking. He was killed.

    Accident: 170388896 - Killed In Fall Between Roof Insulation And Decking

    Employee #1, part of a six-person work crew, was installing a 6 ft long by 25 ft wide section of insulation that had been unrolled across the width of the building, parallel to the leading edge of the finished roof. Employee #1 and three coworkers were sliding the insulation out 6 ft across the purlins to the other crew members, who were working from a 2 ft wide by 3 sheets thick piece of roof decking that was 6 ft from the leading edge. During this process, Employee #1 fell approximately 47 ft between the insulation and the roof deck onto clay. He was killed. Employee #1 was not wearing any fall protection at the time of the accident.

    Accident: 170363071 - Injured In Fall From Bridge

    Employee #1 was working on a bridge under construction when he stepped onto a 5 1/2 in. by 30 in. unsecured piece of metal decking and fell from the bridge. He sustained injuries, but was not hospitalized.
