    Employee #1 was stepping out of his truck at a warehouse after delivering flowers to stores when he suffered a heart attack and died.

    Employee #1 had been delivering newspapers when he was found slumped over his steering wheel. He had died of an apparent heart attack. The coroner attributed his death to natural causes.

    Accident: 170008585 - Dies After Suffering A Heart Attack

    Employee #1, a truck driver, was backing into a bay to make a delivery when a forklift operator found him unconscious, lying on his air horn. CPR was administered, but Employee #1 had died of an apparent heart attack.

    Accident: 170570824 - Face Burned In Diesel Fuel Tank Explosion

    Employee #1, a delivery worker, was unloading 7,800 gallons of #2 diesel fuel into a 100,000 gallon aboveground tank when the tank exploded. He sustained minor burns to his face. The top of the tank blew up in the air approximately 4 ft and came down in its original position, as it was designed to do.

    Accident: 654087 - Killed When Crushed Between Truck And Dock

    At approximately 8:01 a.m. on December 13 1997, Employee #1 had finished his bread delivery at Star Market and was preparing to leave. He apparently saw a semi-tractor truck, owned by Garelick Farms, and thought it was departing from the receiving dock when it was actually repositioning. As Employee #1 jumped down from the dock, the tractor trailer backed up, crushing him against the receiving dock. Employee #1 was killed.

    Accident: 170572739 - Injured When Crushed By Prefab Building

    Employee #1 was performing his normal job duties of delivering prefabricated utility buildings. He was unloading a 10 ft by 10 ft building from a flatbed trailer when his feet slipped from under him and the 1,400 to 1,600 lb structure fell on top of him. Employee #1 suffered fractured ribs and multiple lacerations and scrapes.
