  • Accident: 119947943 - Injured In Fall From Ladder

    At approximately 10:54 a.m. on October 17, 2000, Employee #1, an electrician employed by Thomas J. Buse Electrical Contractor, Riverside, was repairing a parking lot light stanchion in Redlands. After changing the light fixture ballast, he was climbing down the ladder when he fell approximately 8 to 10 ft. He sustained a fractured pelvis. Employee #1 was transported to Arrowhead Regional Medical Center, where he was hospitalized for seven days.

    Accident: 120308267 - Injured In Fall With Purlin Beam

    Employee #1 worked for a heavy construction contractor who built freeway overpasses. He was preparing to descend a ladder to a lower level at an overpass project when he stepped on a purlin beam instead of the ladder rung. The beam was not designed to be a weight-bearing support; it failed under Employee #1's weight and he fell approximately 10 ft. He sustained injuries that included a spine fracture at the L-1 level, two fractured ribs on his left side, a ruptured spleen, and a concussion.

    Accident: 170205876 - Fractures Ankle In Fall While Descending Stairs

    At approximately 1:00 p.m. on January 18, 2000, Employee #1, a mail sorter for Community Vocational Enterprises, was leaving work . She began a search for her identification card while walking down the stairs. She was preoccupied and when she reached the third of four steps, she tripped and fell forward. Employee #1 fractured her left ankle and was transported to the hospital. She underwent surgery that same day, and was released on January 21. No accident-related violations were found in connection with this incident.

    Accident: 14228514 - Injured In Fall When Ladder Rungs Breaks

    Employee #1, age 20, was climbing down a ladder that had been made for the job. A rung broke, and he fell from the second floor into the basement. Employee #1 sustained injuries that required hospitalization.

    Employee #1's foot slipped off the rung of a ladder while he was descending. He fell approximately 5 ft to the ground, sustaining injuries that required hospitalization.

    Accident: 119939304 - Injured In Same-Level Fall Descending Stairs

    At approximately 12:05 a.m. on June 12, 1999, Employee #1, an industrial hygienist district manager for Nestle's Ice Cream, Inc., Bakersfield, CA, was helping the lead operator pull ice cream bars from a table. He was descending the platform stairs, when he slipped and fell on the surface, which had been sprayed with a disinfectant to clean up ice cream residues.

    Accident: 100450766 - Killed In Fall Into New Fuel Tank

    Employee #1, a welder, was found lying on his left side at the bottom of a fuel tank in a newly constructed crew boat. Apparently he slipped while descending and fell against some piping. Employee #1 was killed.

    Accident: 170176747 - Injured In Fall While Descending Ladder

    Employee #1, a contact electrician, was engaged in electrical repairs at a retail furniture showroom/warehouse in Emeryville, California. He began to climb down from a 9 ft high mezzanine in the warehouse using an unsecured 17 ft tall metal extension ladder. Employee #1 fell, fracturing his left leg and knee and dislocating his left elbow. The accident resulted from failure to secure the ladder against displacement, and was aggravated by the employer's failure to train Employee #1 in ladder safety.
