  • Accident: 120126412 - Iron Worker Injures Back In Fall

    At approximately 12:30 p.m. on September 23, 1999, Employee #1, an iron worker employed by Orange County Erectors, Inc., was performing decking activities at a construction site at the California Speedway. He fell approximately 11 ft 3 in. and sustained multiple injuries, including a dislocated back. Employee #1 was transported by paramedics to Arrowhead Regional Medical Center, where he was hospitalized for two days.

    Accident: 170015705 - Two Struck By Falling Wall

    At approximately 11:00 a.m. on September 22, 1999, Employees #1 and #2 and 16 coworkers were raising a 12 ft tall by 60 ft long wall. They were able to raise the wall to an approximately 45 degree angle, but could not raise it into a vertical position because it was too tall. When several of the workers left to get bracing, and the remaining employees were unable to hold up the wall. They let it fall and scrambled back to get out of the way. Employees #1 and #2 were unable to get clear in time and were struck by the falling wall. Employee #1 suffered a dislocated hip and Employee #2 suffered a dislocated shoulder. Both were hospitalized.

    Accident: 201700093 - Injured When Run Over By Golf Cart Trailer

    At approximately 9:00 a.m. on June 13, 1997, Employee #1, of the Broken Top Community Association, was a passenger in a golf-type cart. The cart struck a rock on the road, and he was ejected and then run over by the towed trailer. Employee #1 sustained a broken leg and a dislocated ankle.

    Accident: 170831820 - Injured In Fall When Knocked Off Roof

    Employee #1 was working at the leading edge of a 3 ft screw-down roof. He was knocked off the roof by a 3 ft wide metal roof section blown by a gust of wind. Employee #1 fell approximately 26 ft, landing on freshly spread gravel. He sustained bruises and a dislocated pelvis. Fall protection measures were in place.

    Accident: 170894893 - Finger Injured While Servicing Oil Rig

    Employee #1, a derrickman, was part of a crew that was running rods back into the hole at an oil well service rig. He was holding onto the rod while the floorhand made it up. As the rod rotated, it caught Employee #1's glove and his little finger became wrapped around the rod. He yelled for the floorhand to unscrew the rod, and then came down from his elevated location. Employee #1 was taken in a company vehicle to Westside Hospital, where he was treated for a fractured and dislocated little finger.

    Accident: 201150513 - Injures Ankle In Fall From Roof

    At approximately 8:30 a.m. on April 17, 1997, Employee #1, a laborer employed by F & F Construction, was throwing a bundle of old wooden picket fencing off the roof of a carport. He fell and suffered a dislocated and broken right ankle. Employee #1 was taken by paramedics to Eisenhower Medical Center for emergency medical attention. He spent 2 1/2 days in the hospital and remained under a physician's care, with no estimate of when he would be able to return to work.

    Accident: 200020337 - Injured When Struck By Trailer End Frame

    Employee #1 was struck by the sharp edge of a trailer end frame that was being lifted by a crane. His right thigh was badly lacerated and required 120 stitches to close. He also dislocated his kneecap.

    Accident: 170632491 - Injured When Forklift Drives Off Loading Dock

    Employee #1, of G & G Produce, Los Angeles, CA, was driving a CAT #310 forklift up and down a 5 ft high loading dock while loading pallets of food. He drove too close to the edge when approaching dock #2 from the east and the lift went over the side of the dock. Employee #1 sustained multiple bruises and possibly a dislocated shoulder. He is blind in his left eye, which may have contributed to this accident.

    Accident: 170378814 - Injures Leg And Shoulder In Same-Level Fall

    Employee #1, a retail sales representative for Talbots Toyland, was working behind a sales counter when she tripped on an empty box and fell to the floor. She suffered a fractured femur and a dislocated and fractured shoulder, for which she underwent surgery.
