  • Accident: 170250005 - Killed When Truck Rolls Down Embankment

    Employee #1, age 20, was driving up a hill when he lost control of his truck. He went over an embankment and the truck rolled over several times. Employee #1 was treated by EMS and underwent emergency surgery at Okanogan/Douglas Hospital. Once he was stabilized, he was airlifted to Deaconess Hospital in Spokane, WA, for further treatment. Employee #1 died later that night, having suffered massive blood loss and severe abdominal injuries.

    Accident: 170572762 - Killed When Skidder Pulls Out Stuck Fuel Truck

    Employee #1 was backing a fuel truck over a homemade bridge onto the log deck when he cut too sharp and backed the truck into a ditch. He called the skidder over to pull him out. The skidder operator positioned his machine and Employee #1, in full view of the operator, signaled to start pulling. The skidder began extricating the fuel truck when somehow Employee #1 was killed.

    Accident: 119897155 - Injured In Forklift Accident

    At approximately 1:00 p.m. on March 31, 2000, Employee #1, age 18, inadvertently drove his forklift into the south exterior wall of a warehouse. He suffered injuries to his left foot, for which he underwent surgery.

    Accident: 170041438 - Dies After Suffering A Heart Attack

    Employee #1 was stepping out of his truck at a warehouse after delivering flowers to stores when he suffered a heart attack and died.

    Accident: 170205918 - Injured In Fight With Coworker

    At approximately 4:30 p.m. on March 6, 2000, Employee #1, an assistant manager for San Francisco Honda, was driving a minivan up a ramp and into the tech shop. At the same time, a coworker was driving a company vehicle out of the tech shop and down the ramp. Neither driver yielded, and there was no alternate route for either one. Employee #1 and his coworker got out of their vehicles and confronted one another. When they were face-to-face, the coworker punched Employee #1, who fell onto the ground unconscious. He was later revived and taken by management to Davies Medical Center. The medicine being taken by Employee #1 may have contributed to his aggressive behavior.

    Accident: 14293112 - Killed When Struck By Backing Pickup Truck

    Employee #1 got out of his tractor trailer and was standing 17 ft to the side of a loader that was placing logs onto it. The owner of FDA Logging drove his pickup truck onto the site and past Employee #1 before unloading his crew and equipment. He was backing out of the site when he struck and killed Employee #1.

    Accident: 170007124 - Dies Of Dehydration From Enterocolitis

    On February 6, 2000, Employee #1 was found dead in the sleeper compartment of her truck. The autopsy results stated that she died of dehydration associated with symptomatic; her death was attributed to natural causes.

    Accident: 123299703 - Killed When Run Over By Rolling Tow Truck

    On January 30, 2000, Employee #1, a tow truck driver for Johnny's Auto Body, was trying to extricate a police cruiser from snow. As he hooked up the cruiser and began to move it, the cruiser slid into a telephone pole. Employee #1 then unhitched the cruiser and moved his truck about 30 ft uphill on the street. While the officers examined the minor damage to the cruiser, Employee #1 walked back down the hill toward them. Meanwhile, the truck started to slide down the hill. Employee #1 yelled for the officers to get out of the way as he ran around to the side of the truck. He was pulled under the wheels and killed instantly. It is uncertain if Employee #1 applied the brakes upon leaving the vehicle or if the brakes simply failed to lock the wheels in place. The accident is still under police investigation.

    Accident: 170040737 - Dies After Suffering Heart Attack

    On January 24, 2000, Employee #1 was sitting in the cab of a log truck waiting for his turn to be loaded when he suffered a fatal heart attack and collapsed. Employee #1 was pronounced dead as a result of the heart attack.

    Accident: 170010011 - Dies After Suffering A Heart Attack

    Employee #1 had been delivering newspapers when he was found slumped over his steering wheel. He had died of an apparent heart attack. The coroner attributed his death to natural causes.

    Accident: 170008676 - Dies Of Gunshot To Chest

    At approximately 3:37 p.m. on December 28, 1999, Employee #1, a taxi driver, was shot in the chest. He was taken to Harborview Medical Center, where he died on December 30, 1999 (Medical Examiner's report #99-1499; Seattle police case #99-540549).

    Accident: 170238620 - Killed In Traffic Accident After Heart Attack

    Employee #1 suffered a heart attack while driving a motor vehicle. He was killed in the resulting accident.

    Accident: 170007082 - Dies Of Apparent Heart Attack While Driving

    At approximately 12:10 p.m. on September 30, 1999, Employee #1 was driving when his vehicle left the highway and came to a stop. He died of an apparent heart attack while driving. The New Mexico Medical Examiner's office stated that the cause of death was arteriosclerotic cardiovascular disease.
