    Accident: 801209 - Dies Of Smoke Inhalation And Burns

    Employee #1 was operating heavy equipment to clear land when an employee of another company rode by and saw Employee #1 on fire. He stopped, administered first aid, and called 911. Employee #1 died of smoke inhalation and severe burns. There were no other employees nearby, and there were no burn marks on any of the equipment.

    Employee #1 was moving a Caterpillar over a platform when the vehicle began to fall over the edge. He jumped out but was crushed under the vehicle as it fell over. Employee #1 was killed.

    Accident: 656397 - Leg Injured When Pinned By Excavator

    Employee #1 was measuring the inside of a newly poured foundation wall while standing at the wall's exterior. An excavator was backfilling at the same time, and Employee #1's leg accidentally became pinned between the concrete foundation wall and the crawler's track. He sustained injuries that required hospitalization.

    Employee #1 was killed when he fell out of the bucket of a bulldozer.

    Accident: 100210871 - Killed When Struck By Falling Excavator Bucket

    Employee #1 was working close to where an excavator was using a container to lift a load of dirt. The hydraulic line broke and the bucket fell, striking Employee #1 on the head. He was pronounced dead at the scene.

    Accident: 119947760 - Leg Injured When Loader Overturns

    At 10:40 a.m. on May 8, 2000, Employees #1 and #2, landscapers employed by Blixseth Companies, were riding in the cab of a front end loader as it rode along a golf course access in Rancho Mirage. The loader rolled over and into a concrete-lined storm drain channel. Employee #1 sustained multiple injuries, including a fractured left leg. He was transported by paramedics to Desert Regional Medical Center in Palm Springs, where he was hospitalized for 26 days. Employee #2 sustained minor injuries, for which he was treated but not hospitalized.

    Accident: 119952695 - Injured In Collision Of Scraper And Grader

    At approximately 1:00 p.m. on March 23, 2000, Employee #1, a heavy equipment operator, was working at Badlands Sanitary Landfill. The scraper he was operating collided head-on with a motor grader, and he suffered serious neck and back strain injuries. Paramedics responded to the scene, and Employee #1 was air-lifted to the Riverside County Regional Medical Facility, where he was evaluated, treated, and hospitalized for more than 24 hours. After recuperation, Employee #1 returned to work on March 29, 2000, with no restrictions.

    Employee #1, a laborer at a trash refuse company, was struck by the right rear superstructure of an excavator as it was making a turn. He was killed.

    Accident: 170238638 - One Employee Killed, One Injured When Pinned By Backhoe

    At 6:00 p.m. on January 11, 2000, Employees #1 and #2 were working at a construction site. The operator of a Case 590 SL backhoe was preparing to excavate a small hole. The operator lowered the front bucket and then swung the seat around to face the backhoe controls. He then throttled up, and the backhoe traveled forward, striking Employees #1 and #2 and pinning them against a flatbed trailer. Employee #1 was killed and Employee #2 suffered a broken leg.

    Employees #1 and #2 were run over by a cement truck. Employee #1 was killed and Employee #2 was injured.
