    Accident: 120277157 - Injured In Fall When Scissor Lift Overturns

    Employee #1, a regular employee of a ship builder, was positioning a Skyjack scissor lift, model 3015, in a compartment aboard a ship. Although the lift was to be utilized only on a level surface, the deck of the ship was sloped 3.3 degrees. Employee #1 elevated the scissor lift 15 ft and was running wire through a "wire way" when he moved the scissor lift without lowering it. In doing so, he ran over a 4 in. deck cover plate that was wedged against a 4 in. piece of channel iron being used as a skid rail and that was connected to a portable equipment container. When the right front wheel of the scissor lift ran up onto the channel iron, the combination of the sloped deck and the tire being raised another 4 in. caused the elevated lift to tip over onto its left side. Employee #1 was ejected and fell approximately 15 ft to the deck. He suffered fractures to his hip, left wrist, and spine at the T-12 level. Causal factors in this accident included: (1) The scissor lift was not operating correctly; the speed control had been disabled, allowing extra power to go the unit when it was fully elevated; (2) The area where the equipment was being used was not clear of surface hazards; (3) Employee #1 failed to inspect the unit prior to use, as required; and (4) The employer allowed the unit to be operated on a sloped surface.

    Accident: 170238612 - Killed In Garbage Truck Accident

    On November 10, 1999, Employee #1 was driving a Peterbilt side-loading garbage truck south on Highway 99 when he failed to stop in time and rear-ended a Mazda sedan. The truck veered right and Employee #1 corrected the vehicle's direction by turning left. The centrifugal force pitched him out of the truck and he landed on the pavement. He sustained massive head injuries and died at the hospital the following day. Employee #1 was in the right-hand driving position; he was not wearing a seat belt.

    Accident: 170007090 - Killed When Crushed By Overturned Log Skidder

    At approximately 1:30 p.m. on October 11, 1999, Employee #1 was operating a log skidder when rolled down a hill. Employee #1 was ejected, and the skidder overturned on top of him. He was crushed and killed. Employee #1 was the president of the corporation and owned the skidder.

    Accident: 170006852 - Injured When Brakes Fail In Dump Truck

    At approximately 10:34 a.m. on May 24, 1999, Employee #1 was operating a dump truck when the brakes failed. The truck rolled backward down a hill, then off the roadway and over a steep bank. Employee #1 was thrown from the vehicle and sustained severe injuries.

    Accident: 14293062 - Killed When Struck By Dislodged Board

    Employee #1 was helping a coworker to unjam a board that had become wedged between a guardrail and a conveyor. When the board dislodged, it struck Employee #1 in the head. He was killed.

    Accident: 170238596 - Killed In Vehicle Accident

    Employee #1 was involved in an accident in which his vehicle rolled over. He was not wearing a seatbelt and was partially ejected. Employee #1 sustained a fractured neck, fractured ribs, and a blunt force trauma injury to his head. He was transported to the hospital, where he died.

    Accident: 170074850 - Killed When Pinned Against Wall By Loader Bucket

    Employee #1 and a coworker, both carpenters, were working from the bucket of a loader. They were at a height of approximately 9 ft when the loader operator unintentionally struck the bucket control with his knee. The bucket moved suddenly and both workers tumbled out and became pinned between the loader bucket and a wall. Employee #1 was killed and his coworker suffered abdominal injuries. Use of the loader bucket as a work platform was inappropriate, and neither worker was using fall protection.

    Accident: 14369698 - Killed In Fall From Boom Truck

    Employee #1 was operating an IMT boom, model 1331, serial # 04306779Z, on a truck. The hydraulic cylinder broke (shear failure), and the base of the boom tore free from its structural connection. It struck Employee #1 and knocked him approximately 13 ft from his operator's seat to the ground. He fell on his head and was killed.
