At approximately 2:30 p.m. on February 28, 2000, Employee #1, a supervisor and foreman employed by Ecology Recycling, was attempting to shove an assembly of light steel shelving onto the bed of a 27 ft long box trailer bed. The vertically operated overhead cargo door suddenly fell into the closed position, catching his arm against the floor of the trailer. He sustained a crushed and fractured right arm and elbow and was transported in his own vehicle to San Antonio Community Hospital. Employee #1 was hospitalized for one and a half days.

Accident: 170361430 - Arm Infected After Injury At Fruit Plant
At 12 midnight on September, 28, 1999, Employee #1, who worked as a quality assurance inspector at fruit processing plants, scraped her left elbow on the top of an analyzing table, sustaining an injury similar to a "rug burn." She went to bed at 4:00 a.m. and awoke at 5:00 a.m. to find her elbow swollen. Employee #1 went to her personal physician, who gave her antibiotics for an infection. She also saw the worker's compensation doctor, who increased the dosage of antibiotics. The inflammation continued to worsen, and Employee #1 was admitted to the hospital for intravenous antibiotic treatment. She was released on October. 4, 1999. Although her condition continues to slowly improve, Employee #1 is still experiencing burning and tingling as of April 6, 2000.

Accident: 170363121 - Elbow Injured In Wood Products Plant
Early in the day on August 4, 1999, Employee #1, a mechanic for a medium-sized wood products manufacturer, noticed that his left elbow was inflamed and swollen. He was sent to St. Elizabeth's Hospital, where the elbow was lanced and drained the following day. Employee #1 was hospitalized for two days and treated with antibiotics. Employee #1 spent 80 percent of his work time in the mechanic's shop and the rest at various places throughout the plant. While the date and place where the injury occurred could not be established, the injury was treated as work-related.

Accident: 170361398 - Elbow Injured In Fall At Egg Carton Plant
Employee #1 was the crew leader in the egg carton department at a plant that manufactured egg cartons and other paper products. On February 5, 1998, he was in the basement dye room, refilling the vats that supply dye to the egg cartons being made on the upper level. He was preparing to climb the small flight of stairs to the vat when he slipped on the floor and fell, sustaining a small cut and bruise to his left elbow. He received first aid and was sent back to work. Approximately 12 days later, Employee #1 re-injured his elbow when he bumped it against an egg carton machine wrapper. Within a few hours, the elbow became inflamed and swollen; Employee #1 was hospitalized for four days of treatment.

Accident: 170390165 - Injured When Arm Becomes Caught In Machine Rollers
At approximately 1:00 a.m. on or about January 16, 1998, Employee #1 was operating a Herr Voss coiled metal leveling machine. He had opened the roller set and was checking the driven rollers for dirt when he apparently reached too far into the 2 1/2 in. wide opening with his right hand and was pulled into the moving rollers. He sustained a fractured elbow and contusions and lacerations under his armpit.

At approximately 11:15 a.m. on August 25, 1997, Employee #1, the foreman, was installing 4 ft wide by 8 ft long by 5/8 in. thick pieces of roof sheathing on a house with an 8:12 pitched roof. Employee #1 fell about 9 ft 6 in., landing on an approximately 3 ft high bunk of sheathing. He sustained a dislocated left elbow, a broken right elbow, and torn ligaments in his left knee. Employee #1 was not wearing any personal fall protection equipment, and no guardrail system, safety net system, or kickers/cleats were positioned on the edge of the roof.

Accident: 201090974 - Injured In Fall When Telegraph Pole Breaks
At approximately 9:20 a.m. on August 14, 1997, Employee #1, of Union Pacific Railroad, Roseville, CA, was on an approximately 35 ft high by 13 in. in diameter treated Douglas fir telegraph pole that was between 35 and 50 years old. The line and poles were being taken down. Employee #1 used the metal steps in the pole to climb to cut the wires. The pole broke as he was coming down because it was rotten. He fell, fracturing his elbow, and was hospitalized for a week. Employee #1 had been instructed to check the poles before climbing. He also was told it was not necessary to climb the poles for this job, but just to cut them down. Employee #1 would not talk to the inspector and it was could not be determined if he had checked the pole before climbing it. No citations were issued.
