    Accident: 170250005 - Killed When Truck Rolls Down Embankment

    Employee #1, age 20, was driving up a hill when he lost control of his truck. He went over an embankment and the truck rolled over several times. Employee #1 was treated by EMS and underwent emergency surgery at Okanogan/Douglas Hospital. Once he was stabilized, he was airlifted to Deaconess Hospital in Spokane, WA, for further treatment. Employee #1 died later that night, having suffered massive blood loss and severe abdominal injuries.

    Accident: 170002786 - Killed When Loader Overturns Down Embankment

    At approximately 8:30 p.m. on May 26, 2000, Employee #1 was driving a CAT 988 front end loader when he went off the roadway, down a 20- to 30-ft embankment, and into a gravel pit. The loader flipped as it went over the embankment, landing on its top. Employee #1 sustained massive head and upper body injuries, and died at the scene. The top of the cab was crushed and it took 9 hours to extricate his trapped body from the vehicle.

    Accident: 170235253 - Killed When Vehicle Rolls Down Embankment

    On July 1, 1999, Employee #1, of S & S Logging, was driving a company vehicle into town. He was heading west on Highway 101 when his vehicle crossed the center line and struck another vehicle. Employee #1's vehicle rolled down an embankment, and he suffered blunt trauma to the head. He was killed.

    Accident: 170243703 - Killed When Concrete Truck Crashes

    At approximately 11:00 a.m. on June 30, Employee #1 was operating a concrete pumping truck when he lost control of the vehicle, drove through a STOP sign, and went down an embankment. Employee #1 suffered multiple head injuries and was killed. The police report concluded that the accident resulted from a combination of equipment failure and operator error.

    Accident: 901975 - Killed When Forklift Overturns

    At approximately 8:39 a.m. on June 15, 1999, Employee #1 and a coworker were using a rough-terrain forklift to lift precast stone. Employee #1 was standing on a wooden pallet with the stone while his coworker used a forklift to raise the pallet 22 ft to the edge of the building. The coworker was attempting to reposition the lift and had backed up to the edge of the soil bank when the rear wheel on the driver's side went over the bank. The forklift tipped completely over. The coworker jumped from the cab onto the bank. Employee #1, who was wearing a body harness with a lanyard that was secured to the carriage, was crushed under the forks and carriage. He was killed.

    Accident: 883652 - Killed In Skidder Accident

    Employee #1, age 14, was driving a Garrett skidder, model 21, up a logging road to a logging site. The skidder stalled out on the incline, rolled backward approximately 300 to 400 ft, and plunged off an approximately 50 degree incline. The skidder was not equipped with seat belts, and Employee #1 jumped out, striking his head. He was killed. Employee #1 was not adequately trained on skidder operations.

    Accident: 170006852 - Injured When Brakes Fail In Dump Truck

    At approximately 10:34 a.m. on May 24, 1999, Employee #1 was operating a dump truck when the brakes failed. The truck rolled backward down a hill, then off the roadway and over a steep bank. Employee #1 was thrown from the vehicle and sustained severe injuries.

    Accident: 170132427 - Log Truck Driver Killed When Truck Goes Off Cliff

    At approximately 8:15 a.m. on July 1, 1998, Employee #1 was returning to Anatone, Washington, in an empty log truck. As he proceeded down a steep grade on Highway 82 at Milepost 31, he lost control of the truck at a 90 degree left curve. He crashed through the guardrail and tumbled approximately 1,500 ft to the valley floor. Employee #1 was thrown from the vehicle and died on impact.

    Accident: 170247787 - Killed When Vehicle Goes Off Embankment

    Employee #1 was driving to his work site at an orchard when he failed to negotiate a corner and went off an embankment. His sustained a fractured neck and was killed. Employee #1 had passed through the orchard, but had not yet reported to work.
