    Accident: 170572135 - Dies Of Blood Clot Following Toe Injury

    Employee #1 was riding on the back of a sanitation truck when it backed into a parked car. Employee #1's foot became caught between the truck and the car, and his big toe was crushed. He subsequently developed a blood clot, and died if a massive acute bilateral pulmonary thromboembolism.

    Accident: 200270262 - Dies Of Heat Stress And Pulmonary Disease

    Employee #1, a migrant worker, was detasseling seed corn in a field at a time when the heat index was over 100 degrees F. He was last observed working in the field on a Saturday afternoon, and his body was found the following afternoon. Employee #1 died from a combination of heat stress and chronic obstructive pulmonary disease.

    Accident: 200670750 - Killed In Fall From Ladder

    Employee #1 was on a 14 ft ladder, cutting branches, when a branch broke unexpectedly and knocked him off the ladder to the ground. He was taken to the hospital, where he died on September 26, 1997, from a pulmonary embolism related to the accident.

    Accident: 170064190 - Dies Of Embolism After Being Struck By Falling Load

    Employee #1 was preparing to off load aluminum frames from a flatbed truck, when several bundles of frames fell from the truck bed. One 1,500 lb bundle struck Employee #1 across both legs, breaking them and pinning him to the ground. Employee #1 died of an embolism that resulted from his injuries.

    Accident: 170566947 - Injures Hand Between Clutches; Dies Of Blood Clot

    On December 9, 1996, Employee #1, a crane belt oiler, was working from the side platform of a crane. Because the weather was cold, she decided to get out of the wind and clean the crane's inner components. She opened the outer sliding door, then the inner swinging guard doors, exposing the crane's drums and clutches. She was cleaning with a rag and a spray bottle of mineral spirits when she apparently slipped, and her hand became wedged between two revolving clutches. She sustained serious injuries, and her hand was amputated two weeks later after failed attempts to save it. Employee #1 continued inpatient and outpatient treatment involving skin grafts until January 31, 1997, when she suffered a massive pulmonary thromboembolism (blood clot) that resulted in her death. Employee #1 died at home on the same day that she had been schedulked to have more surgery. All of the crane's guards were in place at the time of the accident,. Employee #1 had received training, and she held a Class B card in the operating engineers' union.
