Accident: 14406995 - Killed When Run Over By Pneumatic Roller
At approximately 11:30 a.m. on May 13, 2000, Employee #1 was working with a crew of approximately 15 men who were repaving a residential street with a diesel engine Dynapac pneumatic roller tire, CP 132, serial #21620241. He was the last person in the paving chain; first was the paving machine, second was the steel wheel roller, and third was the pneumatic roller. The roller came last, allowing the asphalt to cool. It kneaded the oil up to the surface, which actually sealed the top of the asphalt. For some reason, Employee #1 got off the roller and walked to the rear of the machine. The engine was still running, the forward/reverse lever was in reverse, and he emergency stop knob had not been engaged when the roller backed over Employee #1. The back wheels ran over Employee #1's feet first and the front wheels came to rest in the middle of his body, killing him. The roller stopped on its own.

Accident: 122090616 - Finger Amputated When Caught In Rollers
Employee #1, age 20, was cleaning a sheeter machine in preparation for installing new wires. He failed to open the rolls, and his fingers became caught as he was cleaning them with a cloth. The emergency shut off bar had been removed, so Employee #1 could not stop the machine following the accident.

Accident: 510032 - Fingers Amputated In Compactor/Baler
Employee #1, of Sanitary Disposal, Inc., was operating a cardboard compactor/baler. He was removing some extraneous wire from inside the tie enclosure when his hand became entangled in the wire twisters in the bale tie mechanism. He was unable to remove his hand before the tie mechanism cycled, and three of his fingers were amputated. Employee #1 received first aid and was transported by ambulance to the local hospital and then to a larger hospital in Portland. Surgery to reattach his fingers was not successful. Employee #1 did not stop the machine prior to opening the tie enclosure nor did he activate the emergency STOP button on top of the enclosure.

Accident: 896423 - Fingertip Amputated In Saw Blade
Employee #1 was operating the end matcher. He was reaching his right hand into the opening for the stock when it became caught between the alignment lug and the reverse lip. The saw blade amputated the end of his third finger. Employee #1 did not activate the emergency STOP.

Accident: 665620 - Hand Amputated When Caught In Rubber Mill
On August 9, 1997, Employee #1 was being trained to operate a rubber rolling mill. While cutting and mixing the rubber from the mill roll, his glove became stuck to the rubber as he tried to balance himself. He could not free his hand from the glove nor his glove from the rubber. He activated the emergency STOP wire, but his hand went into the pinch point and was amputated approximately 2 in. above his wrist bone.

Accident: 201050556 - Arm Injured When Caught In Machine Rollers
Employee #1, a laminator, had worked for a paper conversion company for six years, in a variety of positions ranging glue man to stacker. The paper conversion company took rolls of paper, laminated two or more sheets together, and then cut them into desired dimensions to make box packaging. Its primary customers were agricultural wholesalers, who used the boxes to hold various types of produce. At approximately 4:00 p.m. on June 30, 1997, Employee #1 arrived for his night shift and upon checking the job schedule discovered that his assignment was to set up a new product on the paper laminator machine. At approximately 5:15 p.m., while he was inspecting the way the paper was running into the rolls and onto the slitters, he lifted the guard in front of the first set of rolls to access the area. The fingers of his right hand became caught by the rolls and was pulled into the machine up to his forearm. A second guard, located at the second set of rolls, stopped his forearm from being pulled in further. Employee #1 was then able to hit the EMERGENCY STOP button with his left hand, and deactivate the machine.

Accident: 14384440 - Dies Of Asphyxia When Crushed On Manlift
Employee #1 was on a manlift. The emergency STOP and safety switches became inoperable, and he could not escape from a step moving upward at 75 fpm and 16 ft above the top landing. Employee #1 was crushed at the head pulley and died of asphyxia. For some reason, the company's safety department had stopped the required weekly inspection of the manlift's up-side safety switches three years before the accident.

Accident: 171045701 - Fingers Amputated In Hay Press
On April 30, 1997, Employee #1 was operating a hay press that was set on automatic mode. The feed arm was not working correctly, so he had to push the bales into the press. As he did so, the feed arm moved faster and pushed another bale into the one on which his hand was resting. This prevented him from pulling out his hand, and his right index and middle fingers were amputated in the press. He called to the production manager, who activated the EMERGENCY STOP. Employee #1 was transported to Meridian Park Hospital for treatment.
