Employee #1, an associate in the garden department at Home Depot store #5211, and a coworker were removing lawn mowers from packing crates. Employee #1 was using the forks of a lift truck to take apart a crate. As he approached the crate, the truck struck and wedged a1 gal jug of muriatic acid (swimming pool grade) against a wooden pallet. The acid was squeezed out of the bottle, splashing onto Employee #1's face and right arm. He suffered first-degree burns to his right eye and on other exposed areas. Employee #1 was transported to Redding Medical Center, where he was treated and released.

Accident: 119824522 - Injured After Inhaling Chlorine
On September 17, 1999, Employee #1 was working in the pool facilities room at Pilgrim Place in Claremont, CA. He inhaled chlorine and sustained chemical burns to his skin, and irritation to his eyes and bronchial passages. CAL/OSHA received the report on September 9, 2000. The employer was cited for a violation of T8CCR 5158, related to confined space regulations.

Accident: 120277686 - Eye Punctured By Flying Nail
On August 11, 1999, Employee #1 was using a shovel-like scraper to remove roofing shingles and materials from the roof of a commercial building at a construction site in Descanso, CA. As he did so, a nail popped up and struck his right eye, puncturing his cornea and lens. Employee #1 was not wearing suitable eye protection for this type of work.

Accident: 896902 - Injured By Chemical Discharge
Employee #1 was removing a resin pin from a fiberglass spray gun so that she could clean and service the respirator. When she pulled back the operational pin, a small amount of trapped air was released, discharging residual resin and methyl ethyl ketone. The material sprayed onto Employee #1's head and face, and some got around her safety glasses and into her eyes. She was transported to the hospital to have her eyes flushed. Employee #1 returned to work two days later, with no apparent adverse effects from the accident.

Accident: 170006654 - Burned And Blinded By Grout Compound
Employee #1 was filling bore holes in rock with an expanding grout compound designed to break the rock. The compound overpressureized the rock and was expelled into Employee #1's face. He suffered chemical burns that resulted in permanent loss of eyesight. He had apparently applied the compound incorrectly and had not been wearing eye protection.

Accident: 170900658 - Injured When Powder Load Triggered
At approximately 8:35 a.m. on September 8, 1997, Employee #1, a laborer for R.E. Lee Design and Construction, Inc., Reseda, CA, went outside the studio building to retrieve some lumber. He was helping to set up for the frame for a platform that was to be built. As Employee #1 was leaving, a hammer fell from his hand and hit a powder load on the ground. The impact triggered the load, and Employee #1 was struck near his right eye. At the time of the accident, he had been working for this employer for five days.

Accident: 171055429 - Eye Injured When Struck By Flying Router Bit
At approximately 5:00 p.m. on August 14, 1997, Employee #1, age 17, of Whitehead Contracting, was using a Roto-Zip router to make an opening in some drywall. The router bit broke off and flew into his eye, resulting in injuries that required hospitalization. Employee #1 was not wearing eye protection although it was available and he had received training on personal protective equipment.

Accident: 202287769 - Killed When Struck In Eye By Flying Piece Of Metal
Employee #1, age 20, was removing a piece of metal banding from around a bundle of scrap aluminum when the piece of metal slipped from his hands and flew out. It pierced through his eye and 6 in. into his head. The Sanford police responded, and Employee #1 was taken to Sanford Hospital, where he was pronounced dead at 4:00 a.m. on July 25, 1997.

Accident: 170572812 - Suffers Respiratory Irritation After Inhaling Fumes
At approximately 5:30 a.m. on July 17, 1997, Employee #1 came on duty to prepare dishes for washing. She added Clorox to several cups and then turned on the dishwasher. After suffering symptoms such as burning eyes and throat, Employee #1 passed out and a nurse was summoned. She was taken to a hospital, evaluated by a doctor, and then released. Employee #1 saw the company physician the next day. That night, she was taken back to the hospital because of shortness of breath, and a lung scan showed irritation from fumes. Sampling of her work area revealed no overexposure or fumes of any level. No citation was issued.
