Accident: 170019343 - Killed When Tractor Overturns During Loading Employee #1, the owner-operator of a farm tractor with a rake attachment, was attempting to load the vehicle onto a flatbed truck. He was driving up two metal ramps when the tractor tipped over and fell off the ramps, trapping him underneath it. Employee #1 was crushed and killed. tractor, falling object, crushed, agriculture, ramp, overturn, loading, farm machinery Accident: 170205173 - Injured When Struck By Falling Beam At approximately 9:45 a.m. on November 9, 2000, employees of the Hathaway-Dinwiddie Construction Company were stripping the deck of 2 by 4 falsework when part of the deck popped loose and knocked an 8 in. by 8 in. by 7 ft beam toward Employee #1. He was standing directly in front of the beam and could not get away in time. The beam fell, striking his head, face, nose, left hand, and knee. Employee #1 was stunned and felt slightly dizzy, but did not lose consciousness. He cut a large white oak tree and, as it fell, a limb became lodged in the top of a hickory tree. The skidder operator took the white oak to the landing while Employee #1 cut down he hickory tree. As it fell, the 42 ft white oak limb was freed. It hit the ground and sprang backward, striking Employee #1 on the head. He was killed instantly. logging, tree, struck by, head, tree limb, slope, kick back, lodged tree, falling object Accident: 119948008 - Injured While Servicing Recreational Vehicle At approximately 5:15 p.m. on October 25, 2000, Employee #1, a recreational vehicle repair technician, was installing a leveling jack on a recreational vehicle when he sustained a serious groin injury involving a ruptured right testicle. Employee #1 drove himself to his home in Riverside, where paramedics were summoned. He was transported to Kaiser Hospital, where he was treated and hospitalized for five days. rupture, automotive repair, mechanic, jack, struck by, falling object, work rules, installing, unsecured, unstable position Accident: 812685 - One Killed, One Injured When Traffic Light Unit Falls Employees #1 and #2 were on an eight-person crew that was installing a traffic signal light with a cross-arm extending over a traffic island. The unit was lifted into position using the boom of a digger truck, and four employees were holding tag guide lines at the outer end of the cross-arm as it was raised. The load line broke, and the traffic light unit fell, striking Employees #1 and #2. Employee #1 was killed and Employee #2 sustained injuries that required hospitalization. falling object, equipment failure, head, struck by, tag line, installing, load line, construction, hoisting mechanism Accident: 101571446 - Killed When Crushed By Falling Tanker Trailer At approximately 11:30 a.m. on September 7, 2000, Employee #1 and the company co-owner were called to a scene where one of their truck drivers was delivering hot emulsion material to a construction site. The tanker became detached from the tractor, and he called the company office to report this incident because the tanker was blocking an active street. A tow service was called to help lift the front of the tanker so that it could be place back on the tractor. It was decided that the tanker was to heavy to lift and that some of the emulsion would need to be removed. A coworker and the co-owner drove to the site with a second tanker, which was going to be used to off-load the damaged tanker. After about half of the contents of the damaged tanker had been emptied, a tow truck attempted to lift the front end so the hot emulsion would flow out the rear outlet. For this process, 4-by-4 wood blocks were placed between the pavement and the tanker to "crib" it. Once the blocks were in place, Employee #1 decided to crawl on his stomach with a chain to try and secure the crane to the fifth wheel pin. As he was crawling under the tanker, his coworkers called, "Don't go under there." He had a jack behind the left front wheel and a block behind the left drive wheel. The jack kicked out from under the forklift , which then fell on his head.
