Accident: 170611941 - Injured By Moving Trailer Wheel Employee #1 was assigned to pick corn on a farm. After the trailer was filled to capacity, he decided to jump on the running stepboard of the trailer and catch a ride to the roadway. The tractor driver was arguing with another coworker. Employee #1 saw the argument getting heated and decided to jump off. At the same time, the driver of the tractor put the equipment in high gear and popped the clutch to accelerate. When Employee #1 jumped off, the accelerated motion threw him against the large trailer tire. Employee #1 collapsed and was transported to the hospital, where she later died. After the argument, the owner told Employee #1 to leave the logging site and to wait in the crummy until the other employees finished their work shift. The loggers normally ended their work shift at about 3:00 p.m. When they were done for the day, they walked over to the crummy, but did not see Employee #1. When they left the site, at approximately 3:30 p.m., they found Employee #1 down the road lying on his back. The crummy driver went back up the hill to the landing site where the owner was working on some equipment. He came down to where the employee was lying. Employee #1 was transported in the crummy to Mercy Health Clinic in Canyonville, OR. He was pronounced dead at the clinic. The temperature that day in Canyonville was 101 degrees F. logging, heat stroke, work rules, fighting, high temperature Accident: 170046387 - Bruised When Wall Falls On Him At approximately 11:00 a.m. on February 10, 1994, Employee #1 was getting ready to load a modular component (a 35 ft wall) when he noticed that the wall was not properly constructed. He confronted the framers, had words, and was quite upset. The employee had to redo the wall himself and in his haste, forgot to install the nuts/washers on the lifting bolts. When he lifted the wall with an overhead crane, the lifting bolts pulled out, causing the wall to fall forward and strike him. He was knocked down, fracturing his left arm. arm, fracture, struck by, workplace violence, fighting Accident: 170128649 - Injured During Altercation With Inmate At approximately 7:00 a.m. on May 15, 1992, Employee #1, a correction officer at the Marion (OR) County Jail, was attacked by an inmate he was attempting to wake up. The inmate pushed Employee #1 against a 42 inch metal railing on the second tier of the pod, breaking the employee's arm. The inmate also sprayed Capstun, a substance used to incapacitate, onto Employee #1's chest and face. fighting, chest, face Accident: 14384101 - Killed When Crushed By Dozer Employee #1 banged his dozer against his coworker's scraper. A harsh argument broke out about the handling and maneuvering of the coworker's scraper. The differences of opinion had been going on for a year and the coworker was tired of Employee #1's treatment. The coworker jumped out of his scraper and went to Employee #1's dozer. A fight started in one of the dozer's tracks. The dozer rolled down the hill and caught Employee #1 in front of the dozer.
