Accident: 119947737 - Injured When Struck By Falling Pipe At approximately 2:15 p.m. on May 12, 2000, Employee #1, a forklift operator and truck loader, was loading sections of steel piping onto a commercial flatbed trailer at the storage and distribution yard. He was adjusting the dunnage between the layers of pipe when a section of pipe became dislodged and fell, striking his right leg. Employee #1 sustained a fractured right femur. Employee #1 had climbed into the bed of his vehicle to adjust or position some of the boxes when he evidently suffered a massive heart attack. He was discovered several minutes later by dock personnel. Paramedics spent approximately 45 minutes trying to revive him, but he was pronounced dead at the scene. He sustained a fractured right ankle and was transported by private vehicle to Redlands Community Hospital, where he was treated and then referred to the Arrowhead Orthopedic Clinic. Employee #1, the son of the owner, was loading a Case forklift onto a flatbed trailer when the forklift overturned on him. The pilings were lifted off the truck by a crane and placed at ground level. The crane was raising a piling slightly, taking the weight off it, when the piling swung around and struck Employee #1 in the back. He was pushed into the track of the crane, sustaining severe chest trauma. The forklift driver was removing bundle #4, and was between 30 and 40 ft away, when a bundle fell off the truck and struck Employee #1. He was driving over a railroad crossing when his truck was struck by a train. Employee #1 was killed. truck driver, flatbed truck, struck by, train, railroad, inattention Accident: 201140365 - Injured When Struck By Falling Concrete Form At approximately 10:00 a.m. on August 19, 1997, Employee #1 and a coworker were unloading metal concrete beam forms from a flatbed truck. The coworker was operating a forklift, and Employee #1 was standing on the flatbed, guiding it. The forklift was picking up one of the forms when it pushed against the form next to it and the whole stack tipped over. Employee #1 saw this happening and jumped 9 ft off the flatbed. At the same time, three of the forms fell to the ground. One struck Employee #1, causing serious injuries to his right leg. He also sustained a broken left leg in his jump off the truck.
