Accident: 170205116 - Fingertip Amputated In Coffee Grinder At approximately 9:37 a.m. on July 6, 2000, Employee #1 was making an espresso drink for a customer when she noticed that there was no espresso in the machine and so switched on the grinder to make some more. The grinder did not start immediately and she lifted the lid to look inside. Employee #1 then stuck her right index finger into the area where the coffee comes out of the hopper. It struck the rotating grinder blade and was amputated at the first knuckle. She was transported to the California Pacific Medical Facility, where she was treated and released. This was Employee #1's third day on the job as counter help. At the time of the accident, she was not under pressure or in a rush to perform her duties. In the process, he amputated a portion of the first digit of his left index finger. His arm became caught by a metal clamp that was holding the rotating part in place ,and his hand was forced into the grinding wheel. The coworker buffed two to three pieces as a demonstration and watched Employee #1 do the same, then left her to work alone. His left hand was too close to the grinding wheel and, when the bar slipped, his hand struck the wheel. He sustained injuries that required hospitalization. grinder, scraper, hand, slip, struck against, grinding machine, grinding wheel, rotating parts Accident: 170572416 - Hand Injured In Power Press Employee #1 was operating an automatic power press to grind burrs off of shaped parts that were dropped onto the conveyor by a robotics extractor. The bottom of the die had a sensor to activate the press when it detected that a part had made contact. Apparently a panel become stuck and did not land properly to make contact. Employee #1 had seen a senior operator go into the die and remove stuck parts, so he attempted to do the same. As Employee #1 tried to get out of the die, he was using his left hand to hold onto the guard part pin for balance.
