    Accident: 170205165 - Injured In Fall From Scaffold

    At approximately 3:15 p.m. on October 18, 2000, Employee #1, an engineer, and a pump operator were using a remote control box to operate a concrete boom pump. Employee #1 was standing on top of a rolling scaffold, assisting laborers in moving the extra length of concrete hose over to the next wall. The boom pump operator was raising the boom slightly to help move the hose when the boom caught the scaffold railing and started to lift it off the ground. Employee #1 jumped off the scaffold, grabbing the top rail of the adjacent wall. The railing broke and he fell backward, away from the wall, striking the back of his head on a concrete slab.

    Accident: 656140 - Injured In Fall From Edge Of Second Floor

    Employee #1 was welding the concrete stop at the edge of the second floor when he fell 22 ft to the ground. He sustained serious injuries, for which he was hospitalized.

    Accident: 170007165 - Injured In Fall From Scaffold

    At approximately 11:15 a.m. on August 16, 2000, Employee #1 fell 14 ft off a scaffold that lacked guardrails. He was transported to the hospital, where he was treated and released.

    Accident: 125526699 - Carpenter Injured In Fall From Balcony

    Employee #1, a carpenter, was part of a crew working on a two-story residence undergoing remodeling. He was re-installing the windows from inside the bathroom but went out to the balcony to tighten some screws because they were easier to reach from there. To gain some height, Employee #1 climbed on top of the railing and knelt on it. He apparently lost his balance and fell 14 ft to the stone yard. Employee #1 sustained serious injuries, for which he was hospitalized.

    Accident: 120280474 - Injured In Fall From Scaffold

    On June 12, 2000, Employee #1 was installing drywall on the interior of the third floor of a commercial building. He had placed a Perry scaffold at the open side of the building within 2 ft of the perimeter. The scaffold did not have the guardrails attached. Employee #1 was working overhead and screwing the drywall in place when he stepped backward off the unguarded end of the scaffold and fell approximately 6 ft onto a concrete floor. He fractured his right arm in two places and fractured his right ankle. Employee #1, an experienced drywall installer, misjudged the hazard of working on scaffolding without guardrails. If guardrails had been in place, he would not have fallen.

    Accident: 837856 - Injured In Fall From Scaffold

    Employee #1 was performing drywall finishing on a ceiling from a tubular welded-frame scaffold. He was leaning over the edge of the scaffold platform to perform finish work above his head when he fell 25 ft from the scaffold to the ground. Employee #1 sustained injuries that required hospitalization. The scaffold was not protected by standard guardrails, and Employee #1 was not using a personal fall arrest system.

    Accident: 170127658 - Fingertip Amputated Between Railings

    Employee #1 was removing the railing from on top of an evaporator shell. He had a chain sling attached to two railings so that he could lift and remove them, but one railing was stuck in its socket. Employee #1 shook the stuck railing and, when it came loose, the other railing lifted up as well. His right little finger became caught between the two railings, amputating the tip.

    Accident: 170571913 - Injured In Fall From Aerial Lift Platform

    At approximately 9:00 a.m. on April 24, 2000, Employee #1 was on a Skyjack elevated work platform, repairing a lift motor for door #5 in the blast freezer. He lost his balance and fell approximately 18 ft to the concrete floor. Employee #1 suffered a concussion and neck injuries, for which he was hospitalized. The guardrail system had been altered by the employer several years earlier, and was 7 in. lower than the required 42 in. minimum height.
