Accident: 951202 - Killed When Crushed Between Forklift And Lumber At 10:45 a.m. on October 15, 1997, Employee #1 had just finished stacking boards on a metal frame at the west end of the dry chain line. While he was straightening the spacing sticks, a forklift operator accidentally hit the accelerator pedal as he was mounting the lift. The vehicle launched forward, crushing Employee #1 against the stack of lumber. He was killed. The vibration of the running engine caused the forklift to slowly move forward, and Employee #1 became trapped at the waist between the dumpster and the hopper. A coworker heard him scream and rescued him. Employee #1 was transported by taxi to a medical center. There, he contracted a bacterial infection that resulted in renal failure. While undergoing surgery, it was discovered that he had suffered colon damage. The pallet was on a moving electrically-powered conveyor system, and Employee #1 was standing near a section where five gridded 4 in. wide steel plates had been installed between 2 1/2 in. diameter rollers, to provide foot access to other parts of the loop. After securing the plastic, Employee #1 was turning to his left to exit when he and lost his balance. In an effort to steady himself, he placed his left hand on the conveyor. His little finger was amputated to the first joint when it was pulled in and became caught between a steel plate and a moving roller. The forklift, which was on the tracks with its forks raised, could not hold the rail cars on the slight incline. The rail cars rolled into the forklift truck, pinning Employee #1. The coupling was ineffective because the hand brake had not been set on the second rail car. Employee #1 jumped on the rail car to set the hand brake. When he realized his effort had failed and the rail car was about to go through the shop doors, he jumped off behind an open side door. The rail car ran into the shop door and knocked it against the employee.
