Accident: 170018022 - Killed When Struck By Falling Tree Employee #1 was on the ground, cutting down a tree, which struck him on the head when it fell. He cut a large white oak tree and, as it fell, a limb became lodged in the top of a hickory tree. The skidder operator took the white oak to the landing while Employee #1 cut down he hickory tree. As it fell, the 42 ft white oak limb was freed. It hit the ground and sprang backward, striking Employee #1 on the head. As the employer was unchaining the Bobcat, he asked Employee #1 to drop the ramps, which he did. The chain on the Bobcat became caught, and the employer concentrated on releasing it and did not notice Employee #1. Another coworker was standing to the right of the trailer watching the employer free the Bobcat. The employer then mounted the Bobcat and said that he was ready to back it off. He looked behind him and did not see Employee #1, so he assumed that he had gone to the office. As the employer began backing up and the rear wheels touched the top of the ramps, the coworker standing to the side screamed for him to stop. Employee #1 was found face down with his head under the right ramp and his feet under the left ramp. He suffered massive multiple fractures and internal injuries to his head, and was killed. The unit was lifted into position using the boom of a digger truck, and four employees were holding tag guide lines at the outer end of the cross-arm as it was raised. The load line broke, and the traffic light unit fell, striking Employees #1 and #2. Employee #1 was standing on top of a rolling scaffold, assisting laborers in moving the extra length of concrete hose over to the next wall. The boom pump operator was raising the boom slightly to help move the hose when the boom caught the scaffold railing and started to lift it off the ground. Employee #1 jumped off the scaffold, grabbing the top rail of the adjacent wall. He sustained injuries that required hospitalization. construction, struck by, head Accident: 120276761 - Injured In Fall Through Roof On October 2, 2000, Employee #1 was working alone on the roof of a commercial building. He was cutting out damaged pieces of the roof, which created approximately 4 ft by 8 ft holes. Employee #1 then removed the damaged material, replaced them, and rebuilt and re-roofed the area. After cutting away a section of the roof, but before he had a chance to remove the damaged area, he stepped onto a previously cut section and fell approximately 21 ft to the floor. Employee #1 sustained fractured ribs on both sides of his body, head trauma including a concussion and a lacerated scalp, a punctured right lung, and a cracked 10th vertebra in his spine. He had been measuring the area where the handrails were to be installed and showing the coworker the area. As he was walking down, he stepped at the edge of a stair, lost his footing, and fell approximately 32 ft onto a concrete floor. The medical examiner's office gave the preliminary cause of death as multiple blunt trauma to the head. construction, fall, handrail, stair, lost balance, head Accident: 119947901 - Injured When Forklift Runs Into Support Beam At approximately 1:50 p.m. on September 8, 2000, Employee #1, a machine and forklift operator, was operating a forklift at a secondary employer's warehouse and manufacturing facility. The forklift struck a structural steel support pole, and he sustained a concussion. Employee #1 was transported by private automobile to Concentra Medical Center in Ontario.
